From this sentence it sounds like some difference in money has to be made up. I think in English you can simply call this payment.

Could be "additional funding" - it has to do with (finding) money. I'll think about it and see if I can come up with other possible translations. A little more context might help.. eg. what is the form (standaardformulier) about?
