Brontekst - Duits Im April 1945 beschloß in Stargard in Mecklenburg ein Papierhändler, seine Frau, seine vierzehnjährige Tochter und sich selbst zu erschießen. Er hatte durch Kunden von Hitlers Hochzeit und Selbstmord gehört.
Im ersten Weltkrieg Reserveoffizier, besaß er noch einen Revolver, auch zehn Schuß Munition.
Als seine Frau mit dem Abendessen aus der Küche kam, stand er am Tisch und reinigte die Waffe.
Er trug das Eiserne Kreuz am Rockaufschlag, wie sonst nur an Festtagen.
Der Führer habe den Freitod gewählt, erklärte er auf ihre Frage, und er halte ihm die Treue. Ob sie, seine Ehefrau, bereit sei, ihm auch hierhin zu folgen. Bei der Tochter zweifle er nicht, daß sie einen ehrenvollen Tod durch die Hand ihres Vaters einem ehrlosen Leben vorziehe.
Er rief sie. Sie enttäuschte ihn nicht.
Ohne die Antwort der Frau abzuwarten, forderte er beide auf, ihre Mäntel anzuziehen, da er, um Aufsehen zu vermeiden, sie an einen geeigneten Ort außerhalb der Stadt führen werde. Sie gehorchten. Er lud dann den Revolver, ließ sich von der Tochter in den Mantel helfen, schloß die Wohnung ab und warf den Schlüssel durch die Briefkastenöffnung.

Er schrie ihnen zu, sie sollten vorangehen. Ihnen folgend, wußte er nicht: hatte er Angst, sie könnten ihm davonlaufen, oder wünschte er, selbst davonzulaufen. Es dauerte nicht lange, und sie waren weit voraus. Als er sie nicht mehr sehen konnte, war ihm klar, daß er zu viel Angst hatte, um einfach wegzulaufen, und er wünschte sehr, sie täten es. Er blieb stehen und ließ Wasser. Den Revolver trug er in der Hosentasche, er spürte ihn kalt durch den dünnen Stoff. Als er schneller ging, um die Frauen einzuholen, schlug die Waffe bei jedem Schritt an sein Bein. Er ging langsamer. Aber als er in die Tasche griff, um den Revolver wegzuwerfen, sah er seine Frau und die Tochter. Sie standen mitten auf dem Weg und warteten auf ihn.
Er hatte es im Wald machen wollen, aber die Gefahr, daß die Schüsse gehört wurden, war hier nicht größer.
Als er den Revolver in die Hand nahm und entsicherte, fiel die Frau ihm um den Hals, schluchzend. Sie war schwer, und er hatte Mühe, sie abzuschütteln. Er trat auf die Tochter zu, die ihn starr ansah, hielt ihr den Revolver an die Schläfe und drückte mit geschlossenen Augen ab.
Er hatte gehofft, der Schuß würde nicht losgehen, aber er hörte ihn und sah, wie das Mädchen schwankte und fiel.
Die Frau zitterte und schrie. Er mußte sie festhalten. Erst nach dem dritten Schuss wurde sie still.
Er war allein.
Da war niemand, der ihm befahl, die Mündung des Revolvers an die eigene Schläfe zu setzen. Die Toten sahen ihn nicht, niemand sah ihn. Das Stück war aus, der Vorhang war gefallen. Er konnte gehen und sich abschminken.
Er steckte den Revolver ein und beugte sich über seine Tochter. Dann fing er an zu laufen. Er lief den Weg zurück bis zur Straße und noch ein Stück die Straße entlang, aber nicht auf die Stadt zu, sondern westwärts. Dann ließ er sich am Straßenrand nieder, den Rücken an einen Baum gelehnt, und überdachte seine Lage, schwer atmend. Er fand, sie war nicht ohne Hoffnung.
Er mußte nur weiterlaufen, immer nach Westen, und die nächsten Ortschaften meiden. Irgendwo konnte er dann untertauchen, in einer größeren Stadt am besten, unter fremdem Namen, ein unbekannter Flüchtling, durchschnittlich und arbeitsam.

Posterior fossada 4 ventrikül minimal belirgindir. Serebellar ve vermian folyolar, bazal sisternalar doğal genişliktedir.
Serebellar hemisferlerde ve beyin sapında patolojik sinyal değişimi izlenmemiştir.
Vasküler yapıların seyri ve akım fenomenleri doğaldır, sağ vertebral arter ince hipoplazik ? görünümdedir. Kranial kafa çiftlerine ait patoloji görülmemiştir.
Sellar ve parasellar bölge yapılan tabiidir, kavernöz sinüsler simetrik ve açıktır. Supratentorial alanda 3. ventrikül ve lateral ventriküller özellikle lateral ventrikül frontal horn ve korpusu anterior hornları düzeyinde belirgin olmak üzere geniş görünümdedir, sağ lateral ventrikül temporal horn ve atriumunda retraktil değişim mevcuttur.
Sağ temporal lob anteromedial ve lateral bölümünde kortikosubkortikal alanda dağınık şekilde 12 sekansta heterojen intens, FLAIR sekansta kortikal bölümleri hipointens, diğer bölümleri intens görünümde eski hemoraji ürünleri içeren kistik ansefalomalazi alanları görülmektedir, bu alanda fokal atrofik değişimlerle sağ lateral ventrikül temporal hornunda retraktil değişim mevcuttur. Her iki frontal lob bazalinde sol frontal lob boyunca konveksite düzeyine dek uzanır şekilde korteksten periventriküler alana uzanımlı, 12 sekansta intens-hafif intens, periferik bölümlerinde yer yer düzensiz eski hemoraji ürünlerine ait hipointansiteler içeren kistik ansefalomalazi alanları izlenmektedir, FLAIR sekansta bu alanların geniş bölümü hipointens kistik nitelikte olup çevresinde gliotik sinyal değişimleri mevcuttur.

Korpus kallozum diffüz şekilde hafif ince görünümde olup özellikle genu ve korpus ön bölümünde volüm azalması görülmektedir, sağ oksipital lobda izlenen enfarkt alanı korpus kallozum splenium sağ yarısına devamlılık göstermektedir.
Sağ temporal kemikte mastoid selüllerde fokal enflamatuar sinyal artışları izlenmektedir. Bilateral timpanik kavite ve solda mastoid selül aerasyonları açıktır.
SONUÇ:
• Kalvariumda frontal ve sağ temporal bölgede post-op değişimler.
• Sol frontal lobun geniş bölümünü kapsar şekilde, sağ frontal lob bazalinde, geniş bölümü kistik nitelikte, eski hemoraji ürünlerine ait hipointansiteler içeren kistik yaygın ansefalomalazi alanları.
• Sağ temporal lob anterolateral bölümünde kortikosubkortikal alanda yaygın kistik ansefalomalazi alanları ve fokal atrofik değişimler, kortikal yüzeylerde laminer nekroz ve hemoraji.
• Sağ oksipital lobun parasagittal bölümünü tümü ile kapsayan ve oksipitoparietal bölgeye uzanım gösteren, kortikal yüzeylerde incelme ve laminer nekroza ait intansiteler izlenen, hemorajik komponenti bulunan PCA sulama alanında kronik enfarkt alanı.

Examination has been performed:
• in TSE T2, FLAIR, GE T2-weighted sequences on axial plane,
• in SE T1-weighted sequence on coronal plane, and
• in SE T1, FLAIR-weighted sequences on sagittal plane.
Changes secondary to the operation undergone are observed in the frontal area of calvarium and in the right temporal region. Structures at craniocervical junction are natural, and foramen magnum is of normal width.
4th ventricle in posterior fossa is minimally remarkable. Cerebellar and vermian folia, and basal cisterns are of natural width.
No pathological signal change is observed in cerebellar hemispheres and the brain stem.
Course and flow phenomena of vascular structures are natural. The right vertebral artery appears to be thin and hypoplasic ?. No pathology in cranial nerves is recorded.
Structures in sellar and parasellar area are normal. Cavernous sinuses are symmetrical and patent. 3rd ventricle and lateral ventricles in supratentorial area appear to be wide, especially at the level of anterior horns of lateral ventricular frontal horn and corpus. Retractile change is recorded in the temporal horn and atrium of the right lateral ventricle.
Cystic areas of encephalomalacia, including old hemorrhagic products, which appear heterogeneously intense in T2 sequence, and cortical parts of which appear hypointense in FLAIR sequence while the remaining parts are intense, are observed to be scattered in corticosubcortical area in the anteromedial and lateral segments of the right temporal lobe. In these cystic areas, focal atrophic changes and retractile change at the temporal horn of the right lateral ventricle are recorded. At the basal of both frontal lobes, cystic areas of encephalomalacia, including hypointensities pertaining to old hemorrhagic products, which appear intense-slightly intense in T2 sequence and irregularly in its peripheral parts, are noted to extend from cortex to the periventricular area through the left frontal lobe convexity. In FLAIR sequence, a larger part of these areas are of hypointense cystic character, including gliotic signal changes in its periphery.
A pathological signal change, which appears hyperintense in T2 sequence and hyperintense-mildly hypointense in FLAIR sequence, is recorded in corticosubcortical area in the parasagittal and occipitoparietal sections of the occipital lobe in the right cerebral hemisphere. Lesion area is of hypointense character in T1-weighted sequence, and signal changes pertaining to the thin hyperintense hemorrhagic component and laminar necrosis are present on cortical surfaces. Corpus callosum is of diffuse character and appears to be slightly thinned, and a volume decrease is observed particularly in the front section of genu and corpus of corpus callosum. Infarct area observed in the right occipital lobe continues to extend to the right half of corpus callosum splenium.
Focal inflammatory signal increases are recorded in mastoid cellules of the right temporal bone. Bilateral tympanic cavity and aerations of mastoid cellules on the left side are patent.
RESULTS:
• Postoperative changes in the frontal and the right temporal area of calvarium.
• Widespread cystic areas of encephalomalacia, including hypointensities pertaining to old hemorrhagic products, the larger part of which are of a cystic character, observed in the left frontal lobe as to cover a larger part of it and in the basal of the right frontal lobe.
• Widespread cystic areas of encephalomalacia and focal atrophic changes in corticosubcortical area in the anterolateral segment of the right temporal lobe, laminar necrosis and hemorrhage on cortical surfaces.
• Chronic infarct area in PCA irrigation zone, including hemorrhagic component, where thinning on cortical surfaces and intensities pertaining to laminar necrosis are observed, which completely covers the parasagittal segment of the right occipital lobe and extends to the occipitoparietal area.
• Ectasia in supratentorial ventricular system, particularly remarkable in lateral ventricular frontal horns, and retractile change in the right temporal horn and atrium.

Amaç ve yöntemler: DPNP’li hastalarda duloksetin 20 mg/gün, 60 mg/gün, 120 mg/gün ile plasebonun ağrı kontrolu açısından karşılaştırılması amacıyla yapılmıştır. 12 haftalık, çok merkezli, çift kör olarak planlanmış olan çalışmaya 457 hasta dahil edilmiştir. Primer etkinlik ölçütü olarak 24 saatlik ortalama ağrı puanında (11 puanlı Likert ölçeğine göre) haftalık değişim oranları değerlendirilmiştir.
Bulgular:
• Duloksetin 60 ve 120 mg/gün, randomizasyon sonrası 1. haftadan başlayıp, 12 hafta boyunca 24 saatlik Ortalama Ağrı Skorunda plasebo ile karşılaştırıldığında istatistiksel olarak daha fazla iyileşme sergilemiştir.
• Duloksetin aynı zamanda sağlık sonuçları da dahil olmak üzere hemen hemen tüm ikincil sonlanım ölçütlerinde plasebodan anlamlı derecede farklılık göstermiştir.
• Duloksetin alan hastalar, plasebo ile karşılaştırıldığında anlamlı derecede daha fazla hasta 24 saatlik Ortalama Ağrı Skorunda %50 azalmaya ulaşmıştır.

Amaç ve yöntemler: DPNP’li hastalarda duloksetin 60 mg/gün, 120 mg/gün ile plasebonun ağrı kontrolu açısından karşılaştırılması amacıyla yapılmıştır. Çalışma, 12 haftalık, çok merkezli, çift kör olarak planlanmıştır. Primer etkinlik ölçütü olarak 24 saatlik ortalama ağrı puanında (11 puanlı Likert ölçeğine göre) haftalık değişim oranları değerlendirilmiştir.
Bulgular:
• Duloksetin 60 mg QD ve 60 mg BID DPNP tedavisinde hızlı etki başlangıcı ile, 24-saatlik ortalama ağrı şiddeti skorundaki düzelme ilk haftadan itibaren plasebodan anlamlı derecede daha fazla olmuştur.
• Duloksetin tüm sekonder ağrı ölçütlerinde (allodini hariç), ortalama değişiklikler açısından plaaseboya göre anlamlı derecede üstünlük sağlamış, ayrıca 60 mg QD ve 60 mg BID arasında anlamlı fark olmadığını göstermiştir.
• Klinik Genel İzlenim Şiddet ve Hastanın Genel İyileşme İzlenimi değerlendirmesi, plasebo ile karşılaştırıldığında duloksetin ile tedavi edilen hastalarda daha fazla iyileşme olduğunu göstermiştir.
• Duloksetin diyabette bir kötüleşmeye neden olmamış ve her iki doz da güvenilir şekilde uygulanmıştır.

• Tedaviden kaynaklanan advers olayların (TEAE’ler) genel insidansında tedavi grupları arasında istatistiksel olarak anlamlı fark bulunmamıştır.
• Duloksetin 60 mg BID ile tedavi edilen hastaların %10’u ya da daha fazlasında bildirilen TEAE bulantı iken rutin tedavi grubunda periferal ödem, kol ve bacaklarda ağrı, somnolans ve baş dönmesi idi.
• Duloksetinin glisemik kontrol, lipid profilleri ya da DPNP gidişatını olumsuz etkilediği görülmemiştir.
• 36 maddelik Kısa Form Sağlık Anketi alt ölçekleri ya da EuroQoL 5 Boyutlu Anketinde istatistiksel olarak anlamlı tedavi grubu farklılıkları gözlemlenmemiştir.

• SF-36 fiziksel bileşen özet skoru ile fiziksel fonksiyon, bedensel ağrı ve hayatiyette duloksetin lehine olacak şekilde tedavi grupları arasında anlamlı farklar gözlemlenmiştir.
Kajdasz DK. DPNP tedavisinde duloksetin: Randomize, çift kör, plasebo kontrollü 3 çok merkezli klinik çalışmanın kanıta dayalı post hoc analiz bulguları

Amaç ve yöntemler: Bu post hoc aanalizin amacı duloksetinin DPNP tedavisindeki ektinliğinin ve tolerabilitesinin NNT (number needed to treat) ve NNH (number needed to harm) bazlı değerlendirmesini yapmaktır. Data, 12 haftalık, plasebo kontrollü, çok merkezli , çift kör 3 duloksetin DPNP çalışmasından elde edilmiştir. Duloksetin 60 ve 120 mg/gün kullanılmıştır. NNT yanıt oranlarına (24 saatlik ortalama ağrıda %30 ve %50’den fazla düzelme), NNH de advers olaylar nedeniyle tedaviyi bırakma oranlarına bakılarak hesaplanmıştır.
Bulgular:
• NNT, 60 mg/gün duloksetin için 5.2 ve 120 mg/gün için 4.9 (%95 CI ve LOCF ile) bulunmuştur
• NNH, 60 mg/gün duloksetin için 17.5 ve 120 mg/gün için 8.8 olarak bulunmuştur
• Bu post hoc sonuçlar duloksetinin DPNP tedavisinde etkin ve iyi tolere edildiğini göstermektedir.
Amaç ve yöntemler: Bu analizin amacı duloksetinin DPNP tedavisindeki yanıt profilini değerlendirmekti. Amaç ve yöntemler: Bu incelemenin amacı duloksetinin DPNP klinik çalışmalarında metabolik parametreler üzerine etkilerini araştırmaktı. Analize benzer desenli 3 akut (12 haftalık) çalışma verileri ile 52 haftalık uzatma fazına devam eden hastaların sonuçları dahil edildi. Esas olarak plazma glukoz, lipid profili ve vücut ağırlığı değişiklikleri araştırıldı. Akut dönemde plasebo grubu, uzatma fazında rutin bakım grubu karşılaştırma amacıyla kullanılmıştır
Bulgular:
• Akut dönemde duloksetin tedavisi orta derecede kilo kaybı (plaseboya göre -1.03 kg; P < 0.001) ile sonuçlanırken, hem duloksetin hem de rutin bakımla uzun süreli tedavi görenlerde anlamlı olmayan kilo artışı görülmüştür.
• Dlx ve PBO’nun gruplar arası farklılıkları bazı lipid parametrelerinde de görülmüş ancak bu değişiklikler genellikle küçük olmuştur.
• Metabolik değişiklikler duloksetinin ağrı şiddetindeki iyileşme üzerine etkisini çok fazla etkilememiş ve sinir iletimi de tedaviden önemli ölçüde etkilenmemiştir.
• Duloksetin tedavisi DPNP hastalarında glisemideki küçük değişikliklerle ilişkilendirilmiştir.
Fishbain DA. Kronik ağrıda ağrı uyku sorunlarına yol açar mı? Duloksetinin DPNP klinik çalışma bulguları

Amaç ve yöntemler: Kronik ağrısı olanlarda uyku problemlerinin sık olduğu bilinmesine rağmen ağrının uyku bozukluğuna ne kadar etkisi olduğu tam olarak belli değildir. Bu raporda duloksetin klinik çalışmalarından elde edilen verilerle bu ilişki araştırılmıştır. Akut dönem plasebo kontrollü DPNP çalışmalarında etkinlik ölçümü olarak, ortalamam günlü ağrı şiddeti, gece ağrısının şiddeti ve ağrı nedeniyle uykunun kesilmesine bakılmıştır.
Bulgular:
• Günde 60 ve 120 mg duloksetin ortalama ağrı ve gece ağrılarının düzelmesinde tedavinin ilk haftasından itibaren anlamlı fark gösterirken, uyku kesintilerinde iyileşmenin değerlendirildiği üç vizitte (Hafta 4, 8 ve 12) plasebodan farklılık göstermiştir.
• Uyku engellenmesindeki değişikliğin ortalama ağrı ve gece ağrılarının şiddetindeki değişikliklerle güçlü bir ilişki içinde olduğu görülmüştür.
• Her ne kadar bu ilişki bir nedensellik ortaya koymasa da bu sonuçlar ağrının DPNP ile ilgili uyku sorunlarına ve belki genel olarak kronik ağrıya yol açabilme ihtimali açısından bir bulgu niteliği taşımaktadır.

Objective and Methods: It is aimed to compare duloxetine versus placebo with regard to pain control in patients with DPNP, when administered at doses of 20 mg/day, 60 mg/day and 120 mg/day. 457 subjects were enrolled into a 12-week, multicenter, double-blind trial. Weekly changes in 24-hour average pain score (based on a 11-point Likert scale) were assessed as the primary efficacy measure.
Findings:
• Compared with placebo-treated subjects, subjects receiving duloxetine 60 and 120 mg/day had statistically greater improvement in 24-hour Average Pain Score for 12 weeks starting from the initial week following randomization.
• Also, duloxetine significantly differed from placebo in almost all the secondary endpoint criteria including health outcomes.
• Compared with placebo-receiving subjects, significantly more number of patients receiving duloxetine achieved a 50% decrease in 24-hour Average Pain Score.
• In duloxetine-receiving group, less than 20% of patients were obliged to discontinue the treatment due to adverse events and duloxetine was tolerated well in this group of patients.

Objective and Methods: It is aimed to compare duloxetine versus placebo with regard to pain control in patients with DPNP, when administered at doses of 60 mg/day and 120 mg/day. A 12-week, multicenter, double-blind trial was planned. Weekly changes in 24-hour average pain score (based on a 11-point Likert scale) were assessed as the primary efficacy measure.
Findings:
• With rapid onset of its effect in DPNP treatment, duloxetine 60 mg QD and 60 mg BID showed significantly greater improvement in the 24-hour average pain intensity score starting from the initial week.
• Regarding all secondary pain measures (excluding allodynia), duloxetine was significantly superior to placebo for average changes and also showed that there is no significant difference between 60 mg QD and 60 mg BID.
• When compared with placebo, the evaluations performed for General Clinical Impression of Intensity and General Impression for Patient's Recovery indicated a better improvement in duloxetine-treated patients.
• Duloxetine did not result in worsening of diabetes and both doses were administered safely.

Objective and Methods: The purpose of this post-hoc analysis is to perform an assessment of the efficacy and tolerability of duloxetine in the treatment of DPNP based on NNT (number needed to treat) and NNH (number needed to harm). Data were obtained from 3 placebo-controlled, multicenter, double-blind trials of duloxetine on DPNP. Duloxetine 60 and 120 mg/day were administered. NNT was estimated considering the response rates (more than 30% and 50% improvement in 24-hour average pain), whereas NHT was figured out considering the rate of patients, who discontinued the treatment due to adverse events.
Findings:
• NNT was found as 5.2 for 60 mg/day duloxetine and 4.9 for 120 mg/day duloxetine (along with 95% CI and LOCF).
• NNH was determined as 17.5 for 60 mg/day duloxetine and 8.8 for 120 mg/day duloxetine.
• These results from post-hoc analysis indicate that duloxetine is effective and well-tolerated in the treatment of DPNP.

Objective and Methods: The objective of this analysis was to evaluate the response profile of duloxetine in the treatment of DPNP. Data from 3 acute period trials of DPNP conducted with duloxetine were included in the analysis, and placebo-controlled doses of 20 mg/day, 60 mg/day and 120 mg/day were administered. At least 30% and 50% improvement in pain was regarded as the response criteria.
Findings:
• Regardless of the selected response criteria, the endpoint response rates in duloxetine-receiving patients were significantly higher compared to those of receiving placebo (60 mg QD or 60 mg BID) in all the three trials.
• Compared with placebo group, the rate of patients in duloxetine treatment group, who did recover from pain, were significantly higher in the visits continuing until the end of the treatment starting from the initial week.
• Response to the duloxetine treatment was likely to emerge during the early stages of the treatment.

Objective and Methods: The purpose of this analysis was to investigate the effects of duloxetine on metabolic parameters in clinical trials of DPNP. Data from 3 acute (12 weeks) trials with similar patterns and the outcomes of patients continuing with the 52-week extended phase trial were included in the analysis. Mainly, changes in the plasma glucose level, lipid profile, and body weight were examined. For comparison purposes, placebo group was used in the acute period, whereas routine care group was used in the extended phase trial.
Findings:
• In the acute period, treatment with duloxetine ended up with moderate weight loss (-1.03 kg compared to placebo; P < 0.001), and non-significant weight gain was recorded in patients treated with either duloxetine and routine care.
• Differences in DLX and PBO between groups were seen in some lipid parameters as well; however, these differences were small.
• Metabolic changes did not affect much the effect of duloxetine on the improvement of pain intensity, and nerve conduction was not substantially affected by the treatment as well.
• Duloxetine treatment was associated with the minor changes in glycemia in DPNP patients.

Objective and Methods: Although it is known that people suffer frequent sleep problems in the presence of chronic pain, it is not quite certain how much does the pain affect the pain level in the sleep disorders. In this report, this relationship was investigated using data acquired from the clinical trials of duloxetine. In the acute period placebo-controlled trials of DPNP, daily average pain intensity, pain intensity during night, and sleep disruption due to pain were considered as measurements of efficacy.
Findings:
• 60 and 120 mg duloxetine administered daily showed a significant difference in the recovery of average pain and of pain experienced during night starting from the initial week of the treatment, and it differed from placebo in the three visits (Week 4, 8 and 12) where the recovery in the sleep disruptions was evaluated.
• It was observed that changes in the sleep prevention had a strong relationship with changes in average pain and the intensity of pain during the night.
• Even though this relationship is not a causal one, these results have the characteristics of a finding regarding the possibility of causing sleep problems related to DPNP and perhaps chronic pain generally.

Objective and Methods: This open-label trial was conducted to investigate whether the efficacy of duloxetine 60 mg/day on the treatment of DPNP continues for 26 weeks. Out of the patients receiving duloxetine 60 mg/day for eight weeks, those of with response continued with the treatment until 26. week, and those of not responding were followed-up having increased the dose up to 120 mg/day.
Findings:
• Patients not responding to the treatment reported a significant decrease in pain when the dose was increased to 120 mg QD.
• In this open-label trial, the effect of duloxetine 60 mg QD was maintained in patients with DPNP within the period of 6 months.

Objective and Methods: Cardiovascular complications are frequently observed in diabetes. In this trial, safety data of duloxetine was investigated in groups with and without CV disease.
Findings:
• In this analysis, it was found that the safety of duloxetine did not show a significant difference between DPNP patients with and without co-morbid cardiovascular disease.
• For each dose of DLX and placebo, average changes in the blood pressure were not statistically significant among patients with and without cardiovascular (CV) disease.
• For each dose of DLX and placebo, rates of hypertension were not statistically significant among patients with and without cardiovascular (CV) disease.

Merkezi Sterilizasyon Ünitesinin 24 saat kesintisiz hizmet vereceği düşünülerek , gerekli personel 12 adet olarak temin edilecektir.
Firmamız tarafından çalıştırılacak Personel Nitelikleri ve Çalışma Esasları
6.1 Yüklenici firma tarafından ünitede çalıştırılacak olan personel sayısı 12 kişi olup , belirlenecek personelin Sağlık Bakanlığı tarafından tarif edilen ve / veya kabul edilen şartlara sahip olması gereklidir.Bu aşamada çalışanlar en az lise mezunu veya tercihen hemşire veya laboratuar teknisyeni / teknikeri veya sağlık memuru veya acil tıp teknisyeni veya sağlık teknisyeni / teknikerleri olacaktır.
6.2 Çalıştırılacak personellerden sorumlu yönetici 1(bir) kişi sağlıkla ilgili teknik meslek yüksek okul mezunu olacaktır.
6.3 Çalıştırılacak personellerden biri biyomedikal teknisyeni; biyomedikal cihaz teknolojisi yüksek okul mezunu veya elektronik meslek yüksek okulu mezunu olacaktır.
6.4 İhaleyi kazanan yüklenici, çalışacak personele ilişkin hastane enfeksiyon kontrol komitesinden onay alacak ve komite gerekli gördüğünde elemanın değiştirilmesini talep edebilecektir.
6.5 Çalıştırılacak personeller emredici hüküm gereği iş kanunu hükümlerine tabidirler. Personel için istenen belgelerin kopyaları noter tasdikli, muhtarlık tasdikli ve kontrollük teşkilatı onaylı olacaktır.
6.6 Merkezi Sterilizasyon Ünitesinde kalite sistemleri gereği yapılacak uygulamaların talimatları oluşturulacak ve Merkezi Sterilizasyon Ünitesindeki uygulamalar eğitim verilerek anlatılacak ve personel bu esaslara göre çalıştırılacaktır.
6.7 Çalıştırılacak personele 3 günlük teorik temel Sterilizasyon Eğitimi verilecek , ünitede temiz, kirli ve steril alan uygulamaları yaptırılacaktır. MSÜ çalışanları teorik ve uygulamalı eğitimden sonra sterilizasyon uygulamalarını gerçekleştirecektir.
6.8 Eğitimlere Enfeksiyon Kontrol Komite üyelerinden katılmak isteyenlerin katılımı sağlanacak, denetim ve kontroller nedeni ile Enfeksiyon Kontrol Komite hemşiresi eğitimlerde bulunacaktır.
6.9 Hastane kontrol komitesinde bulunan Enfeksiyon kontrol hekimi veya mikrobiyoloji uzmanı komitenin belirlediği aralıklarla Merkezi Sterilizasyon Ünitesindeki süreçlerini denetleyebilir.
6.10 Çalışan için aşağıdaki belgeler hazırlanacak bu belgeler ihaleyi kazanan yükleniciden sözleşmenin imzalanmasını müteakip işe başlamadan önce istenecektirve yüklenici tarafından idareye sunulacaktır. Bu belgeler tamamlanmadan sterilizasyon işlemi yükleniciye teslim edilmeyecektir.
1. Bulaşıcı hastalıklar ile ilgili sağlık belgesi: HIV, HBV, HCV, tahlillerinin yapılarak sonucuna göre hepatit B ve tetanoz aşıları uygulanacaktır. Rutin kan sayımı ve biyokimyasal incelemeleri, işitme testleri yapılmalıdır. Sağlık kontrolleri yılda bir kez tekrarlanacaktır.
2. Yılda bir kez her personelin portör taramaları; cilt, göz, solunum sistemi, hematopoetik sistem, nörolojik sistem kontrolleri yüklenici firma tarafından yapılacaktır.
3. Adli sicil kaydı ve sağlık kontrolü yüklenici tarafından yapılacaktır.

6. 4x6 ebadında 2 (iki) adet vesikalık fotoğraf

9. Çalıştırılan tüm çalışanların fotoğraflı panosu hazırlanacak ve sterilizasyon ünitesi sorumlusunun odasına asılacaktır.
10. Çalışanlar 18–45 yaş arası olacaktır.
Personel özlük dosyası:
Her personel için bir özlük dosyası yukarıdaki (6.10 maddesindeki) belgeleri kapsayacak şekilde düzenlenecektir. Bu dosyanın orijinal nüshası Hastane İdaresine verilecektir.
Personel Kıyafeti:
Yüklenici firma, sorumlu uzmanın belirleyeceği kıyafetleri işe başlama gününe kadar temin edecektir. Sorumlu uzman bölümlere göre kıyafetleri farklı renklerde yaptırabilir.
Yüklenici, çalışanlar için hazırlamış olduğu ve idarece onaylanmış sisteme tanımlanmış barkodlu kimlik kartları kullanacak, kartı bulunmayan çalışanlar hastane görevlileri tarafından binaya alınmayacaktır.
Personelin iş giysileri temiz ve ütülü olacaktır. Sterilizasyon işlemi için çalışan personelin giysileri günlük olarak yıkanmalıdır. Kıyafetler kolay ütülenebilir ve ütüsü çabuk bozulmayan kumaşlardan seçilecektir. Yırtık, sökük, kirli ve ütüsüz tespit edilen renklerden daha solgun renkte kıyafet kullanılmayacak. Personele 2 takım iş kıyafeti (forma) ve 2 adet ortopedik, su geçirmeyen, yıkanabilen önü kapalı terlik ve uzun kollu önlük verilecektir.
Uygulamalarda kullanılacak maskeler/ tüm yüzü koruyan siperlikler saçılma ve sıçramalarda kullanıcıyı koruyacak özellikte olacaktır.
Yüklenici çalıştıracağı tüm personelin kıyafetlerinin temizliği, düzeni ve kullanılabilirliğinden sorumlu olup, kullanımla bozulan veya eleman değişikliğinden kaynaklanan ihtiyaçları karşılayacaktır.
Belirlenen kalite ve renkte istenilen model aynen uygulanıp yeniden değişikliğe uğramasına sebebiyet verilmeyecektir. Hizmet yerinde kullanılan kıyafetlerle hastane içinde dolaşılmayacaktır. Hastane içinde kullanılmak üzere uzun kollu laboratuar önlüğü temin edilecektir.
Dekontaminasyon işlemiyle ilgilenen personeller dekontaminasyon işlemi sırasında saçılma ve sıçramalardan korunmak için koruyucu gözlük, sıvı geçirmez uzun kollu önlük, eldiven ve maske kullanmalıdır. Paketlemede çalışan personelin uzun kollu giymesi uygun olacaktır.
Kullanılacak eldivenler kesici delici alet yaralanması riskine karşı daha kalın ve bilek kısmı daha uzun olacaktır.
Personel Hijyeni:
Çalışma esnasında personel bireysel temizliğine özen gösterecektir. Erkekler saç ve sakal uzatmayacaklardır.
Günlük sakal ve bıyık tıraşı olacaklar, uzun saçlı bayanlar saçlarını toplayacaklar ve boneden taşmasına izin vermeyecekler.
Ellerinde cerahatli yara, bere, yanık ve benzeri bulunan personel çalıştırılmayacaktır.
Personel bone kullanacaktır, kirli eldivenle temiz yüzeylere temas edilmeyecek, bu temiz yüzeylere hiçbir surette insan atıkları ve sıvıları bulaştırılmayacaktır.
Bu sarf malzemelerini yüklenici firma temin edecektir ve bu sarf malzemeler için ek bir ücret talep edilmeyecektir.
Personel Çalışma Planı
Günlük ve haftalık olarak aşağıdaki gibi olacaktır:
a- Günlük Sterilizasyon İşlemleri:
1- Sterilizasyon ünitesine hastanemizin ameliyathanelerinden ve ihtiyaç duyulabilecek bölümlerinden gelen cerrahi set, ameliyat setleri, pansuman setleri veya diğer malzemelerin hangi servise ait olduğu, içerik listesi yapılarak imza karşılığı ve bilgisayar onaylı teslim alınacak ve teslim edilecektir.
2- Sterilizasyon ünitesine gelen malzemelerin ve cerrahi aletlerin yıkama ünitelerinde yıkanması ve kurutulması, paketlenmesi, uygun sterilizasyon yöntemi ile steril edilmesi ve istenildiğinde ilgili üniteye gönderilmesi sağlanacaktır.
3- Birden fazla parçadan oluşan malzemelerin bağlantıları sökülmeli, kilitli veya kapalı olan malzemelerin ağzı açık olarak yıkayıcıya konmalıdır.
4- Malzemeler paketlenmeden önce durulanmalı ve tamamen kurutulmalıdır. Gerekirse alet tepsisinin tabanına nemi alması için havlu yerleştirilmeli, kenarları aletlerin üzerlerine kapatacak büyüklükte olmalıdır.
5- Her sabah buhar otoklavların Bowie-dick ve kaçak testleri yapılacaktır. Uygun sonuç alındığı sterilizasyon sorumlusu tarafından onaylanacaktır.
6- Malzemelerin paketlenmesi ayrı ve temiz bir yerde yapılmalıdır. Alet tepsisi bohça ve paketleme işlemi standartlara göre (mektup veya dikdörtgen şeklinde) ve açılmayacak şekilde paketlenmelidir.
7- Buhar otoklavı için biyolojik indikatörler, her gün ilk çevrimde, cihazın bakım ve tamirinden sonra ve implant içeren set varsa bu setin bulunduğu her çevrime mutlaka konulacaktır. Bu işlemin kimin tarafından yapıldığı, uygulama saati, sterilizatör numarası ve test sonucu gibi kriterler otomatik kayıt altına alınacaktır.
8- Etilen oksit sterilizatörün biyolojik testleri her çevrimde yapılacaktır.
9- Yıkama cihazlarının yıkama etkinlik kontrolleri yapılacaktır..(Kan, Protein indikatörleri vb)
10- Ambalajlama esnasında delici ve kesici uçlara önlem alınmalı, otoklava yerleştirilen ağır malzemeler alt tarafa yerleştirilmeli, hafif ve hassas malzemeler üste konmalıdır.
11- Ambalaj sterilizasyon yöntemlerine göre ayrılmalıdır.
12- Sterilizasyona girecek her paket içine kimyasal indikatör konacaktır.
13- Gerek tekstil gerek wrap kâğıtlarla yapılan paketlemelerde malzemelerin boylarına göre ambalaj seçilmeli, standartlara uygun paket boyları ve ağırlıkları olmalı ve ambalaj malzemesi çift kat sarılı olmalıdır.
14- Otoklav bandı paketin rahat görülebilir yerine yapıştırılmalıdır.
15- Ambalaj üzerine paket içeriğinin tamamı, hangi servise ait olduğu, paketi düzenleyenin adı soyadı, paketin sterilizasyon tarihi ve şekli, son kullanım tarihi ve kontrol numarası gibi içerikler belirtildiği etiket yapıştırılacak ve aynı etiket ameliyathane hemşireleri tarafından ameliyat defterine yapıştırılacaktır. Ayrıca bu işlemler bilgisara otomatik kayıt edilmelidir.
16- Paketlerin sterilizatöre konması standartlara göre olmalıdır.
17- Konteyner filtrelerinin düzenli olarak değiştirilecektir.
18- Yüklenici ameliyathane için gerekli olan spanç, fındık tampon, ped, gazlı kompres pedi gibi malzemelerin sterilizasyona gönderilmesi işlemlerini de yapacaktır.
19- Sterilizasyondan çıkan ambalajlar kullanım öncelik sırasına göre ve ilgili servisin kullanımına göre raflara konacaklardır.
20- Cerrahi setlerin içerisindeki aletlerin bakım ve yağlama işlemleri ihtiyaç olduğu fark edildiğinde uygun solüsyonlarla bakımları yapılmalı ve bu işlemlerin kaydı tutulmalıdır.
21- Sterilizasyon ünitesine gelen cerrahi malzemelerde kırılma, aşınma, paslanma, korozyon, körlenme veya hasar görme durumu ya da yırtık, sökük, kirli veya yamalı tekstil malzemeler, teslim eden kişi ile karşılıklı tespit edilecek ve ilgili birime bilgi verilecektir.
22- Ünitede bulunan tüm cihaz ve malzemelerde ortaya çıkabilecek arıza, zarar veya kayıptan yüklenici sorumlu olacak ve zarar yükleniciye ait olacaktır.

1- PLACE OF SERVICE PERFORMANCE:

2- NAME OF THE WORK: Marmara University Training and Research Hospital Central Sterilization Unit Duty of Receiving Service
3- SCOPE OF THE WORK: Includes the service of receiving all the devices and equipments after having carried out an inspection, which are to be submitted by all the departments of the Marmara University Training and Research Hospital, such as the operating room and the emergency service wherein invasive interventions are performed besides the operating room, delivery room, intensive care units, catheter laboratory, interventional radiology unit, polyclinics, inpatient floor, in vitro fertilization unit, etc., washing and decontaminating of those, control and maintenance, packaging, sterilizing, storage, delivering these to the relevant units upon request, and carrying out relevant services by the specified technical staff within the scope of established criteria.
4- PLACE OF DUTY: The service area includes the Central Sterilization Unit in our hospital.

Considering that the Central Sterilization Unit will offer a 24-hour uninterrupted service, 12 necessary personnel will be provided.
Quality of Personnel to be Employed in our Company and Rules of Procedures
6.1 12 personnel shall be employed in the unit by the contractor company. Personnel to be determined should conform to the requirements described and/or accepted by the Ministry of Health. Employees at this level shall be graduated at least from a high school; shall be a nurse, laboratory technician, health officer, emergency medical technician or health technician.
6.2 1 (one) manager responsible for personnel to be employed shall be graduated from a technical vocational high school of health.
6.3 Among the personnel to be employed, one of them shall be a biomedical technician and graduated from a high school of biomedical device technology or from an electronic vocational high school.
6.4 Contractor that won the tender shall obtain approval from the hospital's infection control committee regarding the personnel to be employed and the committee may request personnel replacement if required.
6.5 Personnel to be employed are liable to the labor law provisions as required by the imperative provisions. Copies of the documents requested from the personnel shall be notarized, and approved by the village headman and an inspection authority.
6.6 Provision of implementations, which are to be carried out in the Central Sterilization Unit as required by quality systems, shall be generated; a training shall be offered regarding the implementations in the Central Sterilization Unit and personnel shall be employed according to these procedures.
6.7 Personnel to be employed shall be given a theoretical basic Sterilization Training and practices in clean, dirty and sterile areas shall be performed in the unit. CSU staff shall realize sterilization implementations following the theoretical and practical training.
6.8 Participations of those members of Infection Control Committee who are willing to be involved in trainings shall be ensured and a nurse from the Infection Control Committee shall attend the trainings.
6.9 An Infection Control Physician or Microbiologist taking part in the hospital's control committee may inspect processes in the Central Sterilization Unit at intervals specified by the committee.
6.10 Following documents shall be prepared for the employee. These documents shall be requested from the contractor that won the tender prior to the start of the work at that time following the signing of the contract and submitted to the administration by the contractor. Sterilization process shall not be consigned to the contractor unless these documents would have been completed.
1. Bill of health related to the infectious diseases: HIV, HBV and HCV tests shall be performed. According to the results, vaccines of hepatitis B and tetanus shall be administered. Routine blood count and related biochemical analyses, and hearing tests should be performed. Health check-ups shall be repeated once a year.
2. Carrier screenings and check-ups for skin, eye, respiratory system, hematopoietic system, and neurological system shall be performed by the contractor company for any personnel once a year.
3. Criminal record and health check-up shall be undertaken by the contractor.

9. A panel with photos of all the employed personnel being attached onto it shall be prepared and posted at the wall of the sterilization unit officer.
10. Employees shall be between ages of 18–45.
Personal file of the employee:
A personal file shall be arranged for each employee as to include the documents above (in the item 6.10). Original copy of this document shall be provided to the Hospital Administration.
Employee Clothing:
Contracted company shall supply the clothing specified by the contact authority until the starting date of the employment. Contact authority may have the clothing made in different colors tailored to the departments.
Contractor shall use identity cards with barcode, which have been prepared for employees and defined to the system approved by the administration. Personnel with no cards shall not be allowed to enter the building by the hospital officials.
Work clothing of the personnel shall be clean and ironed. Clothing of the personnel involved in the sterilization process should be washed on a daily basis. Clothing shall be selected among durable press fabrics, which could be ironed easily. No clothing color detected to be paler than those of being torn, split, dirty and non-iron clothing shall be used. Personnel shall be provided with 2 suits of work wear (uniform), 2 orthopedic, water-resistant, and washable closed slippers and long sleeve apron.
Masks/visors protecting the whole face, which are to be used in the practices, shall protect the user from dispersion and splash.
Contractor shall be responsible for the cleaning, layout, and availability of the personnel to be employed and meet the needs arising due to deformation by usage or personnel change.
The model in indicated quality and color shall be applied identically and it shall not be allowed that it shall undergo a change once again. No walking around in clothing worn in the service area shall be permitted. Long sleeve laboratory apron shall be provided to be used within the hospital.
During the process of decontamination, personnel responsible from the relevant process should use eye protection, liquid tight long sleeve apron, gloves, and mask to be protected from dispersion and splash. Long sleeve clothing shall be appropriate for the personnel involved in the packaging process.
Gloves to be used shall be thicker and their wrist parts shall be longer against the risk of sharp object injury.
Personnel Hygiene:
Personnel shall take care of his/her personal hygiene during work. Male personnel should not grow hair and beard.
They shall shave daily. Women with long hairs shall tie their hairs up and should not let them getting out of the bonnet.
Personnel having purulent wound, bruise, burn, and alike in their hands shall not be employed.
Personnel shall use bonnet. No contact with clean surfaces shall be allowed by dirty gloves and the contamination of these surfaces by human waste and fluids shall not be permitted to happen.
These consumables shall be supplied by the contractor company and no additional fee shall be demanded for these.
Working plan of Personnel
The plan will be according to the following on a daily and weekly basis:
a- Daily Sterilization Processes:
1- It shall be determined that surgical set, surgical operation kits, wound dressing kits or other equipments, which have been delivered to the sterilization unit from the operating rooms and the other departments of our hospital that may be in need of those, are submitted by which service. A list of content shall be prepared and these shall be received by signature and by a digital signature, and submitted.
2- It shall be ensured that the equipments and surgical devices received by the sterilization unit shall be washed and dried in washing units, packaged, sterilized by an appropriate sterilization method, and delivered to the relevant unit if required.
3- Equipments made up of more than one parts should be disassembled, and locked or closed equipments should be placed into the washing unit with open mouth.
4- Equipments should be rinsed prior to packaging and dried completely. If needed, a towel should be placed onto the floor of the device tray for dehumidification, and its sides should as large as to cover up the devices.
5- Every morning, Bowie-dick and leakage tests shall be performed for steam autoclaves. The appropriate result shall be approved by the person responsible for sterilization.
6- Packaging of the equipments should be done in a separate and clean place. Device tray should be packaged according to the standards for bundle packing and packaging process (in an envelope or rectangular shape) as not to be opened.
7- Biological indicators for steam autoclave shall be definitely placed at the initial cycle following the maintenance and repair of the device, and at every cycle where a set is included, if any. Criteria such as who performed the process, procedure time, sterilizer number, and test result shall be recorded automatically.
8- Biological tests of ethylene oxide sterilizer shall be performed in every cycle.
9- Washing efficiency of washing devices shall be checked (Blood, Protein indicators, etc.).
10- During packaging, measures should be taken against drilling and cutting edges. Heavy equipments placed into the autoclave should be laid on the bottom, and light and fragile equipments should be placed up onto them.
11- Package should be divided with respect to sterilization methods.
12- A chemical indicator shall be inserted into any packages to enter the sterilization process.
13- In packaging processes accomplished either by textile or wrap papers, packaging should be selected in accordance with sizes of the equipments. Package sizes and weights should be suitable to the standards, and the packaging material should be double-wrapped.
14- Autoclave band should be labeled onto an easily visible place.
15- A label indicating contents such as entire package content, which service it belongs to, name and surname of the person preparing the packaging, sterilization date and method of the package, expiration date, and control number shall be labeled onto the package and the same label should be glued into the surgical procedure logbook by the surgical nurses. Additionally, these procedures should be recorded to the computer automatically.
16- The placement of packages into the sterilizer should be in accordance with the standards.
17- Container filters shall be replaced regularly.
18- Contractor shall also perform the procedure of submitting equipments for sterilization required in the operating room such as sponge, hazel pad, pad, and gauze dressing pad.
19- Packages completed the sterilization process shall be placed into shelves according to their order of priority of use and to the use of the relevant service.
20- In case that it is noticed that the devices within the surgical kits require maintenance and lubrication, their maintenance should be performed using appropriate solutions and these procedures should be recorded.
21- Fracture, erosion, oxidation, corrosion, dulling or damaging of the surgical equipments or torn, split, dirty or patched textile materials dispatched to the sterilization unit shall be determined in collaboration with the person who submitted them, and the relevant unit shall be informed about.
22- Contractor shall be responsible for the failure, damage or loss that may occur in any of the devices and equipments in the unit, and the damage shall be undertaken by the contractor.

This mail is to introduce you to the concept of “Contract Catering”. We have been in discussions with Garrets International, who are a contract catering company to help us provide good quality provisions to your vessel but also at a good and reasonable price.
What is “Contract Catering”?
Instead of the traditional way of managing victualling, between the ship and the local chandlers or through our purchasing department, all requirements for provisions are forwarded direct to the catering contractor (in this case Garrets). Garrets then arrange supply through their local, preferred Chandlers and liaise with local Agents for delivery to the vessel, utilising existing launch arrangements wherever possible. Garrets will use their local knowledge to arrange the most cost effective supply and will recommend any changes in the requisition, based on price, known quality and availability at a particular port as well as stock levels on board. The Master is allowed a small amount of cash in any one calendar month – to cover any emergency purchases.

What are the advantages of Contract Catering?
1/ It must be stressed that this is not simply about saving money – by using the services of a contract caterer we expect to be better able to control expenditure within budget, but no reduction in the existing budget is anticipated. A feeding rate has been agreed with Garrets ($9 per person, per day) and it will be expected that you remain within this rate.

3/ More effective handling of disputes with direct suppliers – given Garrets’ purchasing power they are better positioned to ensure that Chandlers only deliver good quality provisions. In case of any problems Garrets will support us in addressing these.

6/ An “expert” point of contact and personalised support service for the Masters
7/ Advice on diets and healthy eating

I've been working as a medical freelance translator for 12 years, and have extensive knowledge in the field of medical translation (informed consent forms, case reports, biochemical texts, patents, medical devices, pharmaceuticals, epicrisis, surgical operative reports, clinical studies, laboratory examinations, all kinds of radiology reports including Chest X-Ray, Upper or Whole Abdominal CT or USG, Thoracic CT, etc, Electrocardiography, Audiology, EEG, Pathology, etc. These are the ones which I could recall at the very moment. As I stated in my CV, I studied medicine for 3 years, which is vital, I think, in providing proper translations that do require medicinal knowledge apart from being a reliable translator with good expressive skills.
