Curative services may be considered from the angle of logistics i.e., the series of "operations" leading from the whole population to a selected group, viz, the cured patients. The operational efficiency of the service can then be expressed in terms of the outputs of work, and of the probabilities of coverage achieved by each successive "operation". In other words: an individual may be a "case" of tuberculosis or not. He may - or may not - live in a zone where health services operate effectively. He may - or may not - seek medical advice if urged by chest symptoms. His disease may be efficiently diagnosed - or not. He may be successfully treated - or not. What are the chances that a case of tuberculosis, in a given situation will proceed right up to the end of the line, where he may be considered to have been successfully treated? This approach involves an analysis of that situation and the formulation of a simplified model to represent it wherein a series of interlocked subsystems are expressed quantitatively by a limited number of epidemiological, sociological, administrative, operational and technical parameters. The object of this systems analysis is to determine which of the parameters can be expected to exert the greatest influence over the total efficiency of the system in a given situation, so as to guide the planning and implementation of the services (investment of resources), and to serve as a forecast of efficiency against which the observed performance in the course of implementation will later be measured. See Appendix I.
Each subsystem and its parameters are now introduced in detail.
Caseload:
The basic epidemiological parameters that are relevant to the present systems analysis are the clinical, bacteriological or radiological evidence of tuberculosis in individuals, and the probability with which such evidence would be observed under ideal conditions in an individual at a point of time. A description of the curative services depends therefore essentially on the basic definition of the word "case". From the epidemiological standpoint, persons excreting tubercle bacilli transmit infection. Let these persons be termed "cases of tuberculosis". By what criteria can one identify them?
The analysis of the material collected in WHO-assisted tuberculosis surveys provides an indirect answer to this question. In a study reported by Roelsgaard (1964), various definitions of a "case of tuberculosis" have been assessed in terms of the probability of transmission - as estimated by the prevalence of infection among young household contacts of the "case". It was found that when:
a. persons excreting tubercle bacilli in a single sputum specimen, and
b. persons who, in the absence of bacilli in the first sputum specimen, present a positive tuberculin test and one or more cavities on X-ray,
are defined as "cases", and only so, the risk of infection among children contacts is significantly (actually three to five times) higher than among children presumably without any such household contact.
These persons may therefore be considered as genuine cases of tuberculosis for the purpose of this paper and not much would be gained by adopting a more liberal definition. It is therefore proposed to retain the above epidemiolo¬gical definition as the basis for the following analysis.
If we note α1 for the "state of health", α1 may denote the presence of bacilli in the first sputum specimen ando α2 the presence of a cavity and of tuberculin positivity, but without bacilli. We can express that the probability of an individual being a "case of tuberculosis" Pr(TB) = Prα1 Prα2. At any point of time the sum of individuals with one and/or the other findings constitutes the caseload. The rest of the community consisting of individuals without bacilli or cavities are presumed to be non-tuberculous. They are referred to in the fallowing as the "community" or the "public". Let's α3 denote their state of health.

En una muestra de 108 adultos mayores de 60 años que recibieron asistencia en la Facultad de Odontología de la Universidad Nacional del Nordeste, se determinó el estado de salud dental y periodontal y la asociación de la cobertura de seguridad social en atención de salud con la necesidad de prótesis.
El 57% eran mujeres; 66% tenía entre 60 - 69 años. 82% poseía obra social. Índice CPO-D promedio 25,31 - componente perdido promedio 22,13 (69,13%). Se halló ausencia de enfermedad periodontal en 93,26% y 40% presentó O’ Leary no compatible con salud. Pacientes sin prótesis superiores 63,89% y 83,33% inferiores. Necesidad de prótesis parcial inferior 42,59% y 25% superior, completa en el arco superior 34,26% y en el inferior 33,33%.
De los sujetos con cobertura en salud 62% necesitaban prótesis inferior y un 41% superior. En quienes no poseían cobertura en salud los porcentajes fueron menores, 20% y 18% respectivamente.

In a sample of 108 adults over 60 years of age who received assistance in the College of Dentistry of the Northeastern National University, a determination was made about the dental and periodontal health and the association of social security coverage on health care when dentures were needed.
57% were women; 66% were between the ages of 60 and 69. 82% had medical insurance. Overall DMFT index of 25.31 – average missing component of 22.13 (69.13%). Absence of periodontal disease was found in 93.26%, and 40% presented O’Leary not compatible with health. Patients without top prosthesis, 63.89%, and bottom, 83.33%. Need of partial bottom prosthesis, 42.59%, and top, 25%, full prosthesis on top arch, 34.26%, and on bottom arch, 33.33%. 62% of the people with health insurance needed bottom prostheses, and 41% top ones. Among those who did not have health insurance, the percentages were lower, 20% and 18% respectively.

Resumen:
En el campo de la restauración dental, la adhesión a la dentina ha evolucionado en los últimos años. Los cementos resinosos autoacondicionantes fueron diseñados para reducir el número de pasos clínicos, eliminando la necesidad de grabar, aplicar un primer y un adhesivo por separado en diferentes etapas. Esto, además de ahorrar tiempo reduce en gran medida la posibilidad de cometer errores durante la aplicación del protocolo de adhesión. Para la resolución de los casos clínicos se utilizó el cemento autoacondicionante Rely X U 100 (3M-ESPE) color A2 (escala VITA).
El objetivo es abordar las recientes tendencias en odontología adhesiva, dado la velocidad con que se produce el avance en la actualización del conocimiento en esa área.

Summary
Adherence to dentin has evolved in the past years in the dental restoration field. Self-adhesive resin cements have been designed to reduce the number of clinical steps, eliminating the need for etching, priming and bonding steps. Besides saving time, this method greatly reduces the chances of making mistakes during the bonding protocol procedure. Shade A2 (VITA scale) RelyX U100 (3M-ESPE) self-adhesive cement was used for the resolution of the clinical cases.
The objective is to address new tendencies in adhesive dentistry, given the speedy progress in the modernization of this field’s knowledge.
