1. What XXXXX is and what it is used for
XXXXX is a counteragent (antidote) for a specific group of medicines called benzodiazepines. Benzodiazepines have sedative, sleep inducing and muscle relaxing properties. They are used to make you sleepy and tranquillize you if you are anxious. XXXXX may completely or partially reverse these effects.
XXXXX may therefore be used
• in anaesthesia to wake you up after surgery or certain diagnostic tests
• if you have been held under sedative conditions in intensive care.
XXXXX may also be used for the diagnosis and treatment of intoxications or overdose with benzodiazepines.
XXXXX is used in children older than 1 year to wake them up after they have received benzodiazepines to make them sleepy during a medical procedure.
2. What you need to know before you use XXXXX
Do not use XXXXX
● if you are allergic to XXXXX or any of the other ingredients of this medicine (listed in section 6).
● if benzodiazepines have been administered to you to control a potentially life-threatening situation (for example control of pressure in the brain or a serious epileptic seizure).
Warnings and precautions
Special care has to be taken
● if you are epileptic and have received benzodiazepine treatment for a long period of time. In this case the administration of XXXXX can cause seizures.
● if you have a serious brain injury (and/or instable pressure in your brain) as XXXXX can cause an increased pressure in your brain.
● if you suffer from liver disease. Your doctor will carefully adjust the dosage of XXXXX.
● if you have experienced panic attacks in the past as XXXXX can cause new attacks.
● if you are very nervous about having your operation or have a history of anxiety. Your doctor will carefully adjust the dosage of XXXXX.
● if you have been treated for long periods with high doses of benzodiazepines as there is a risk of withdrawal symptoms (Withdrawal symptoms are listed in section 4. “Possible side effects”).
● if you are dependent on alcohol or medicines. In this case you have a higher risk of benzodiazepine tolerance and dependence.
● if you have a coronary heart disease. Your doctor should be informed as she / he might decide to keep you longer under sedation.
Your alertness and vital signs (like blood pressure, heart and breathing rate) will be controlled for an adequate period of time after you received XXXXX. As the action of XXXXX is usually shorter than that of benzodiazepines, sedation may recur. You will be closely observed, possibly in the intensive care unit, until the effects of XXXXX have gone away.
If XXXXX is administered to you at the end of your operation to wake you up, it should not be given until the effects of muscle relaxants have gone away.
Your doctor will consider postoperative pain before giving you XXXXX.
If you do not wake up after XXXXX is administered, another reason for this will be considered because XXXXX specifically reverses the effects of benzodiazepines.
Your doctor will avoid rapid injection of XXXXX. If you have received long-term (chronic) treatment with benzodiazepines, rapid injection of high doses of XXXXX (more than 1 mg) may cause withdrawal symptoms.
XXXXX is not recommended for the treatment of benzodiazepine-dependence or benzodiazepines-withdrawal-symptoms.
Your doctor will administer XXXXX only with particular caution in case of mixed intoxications with benzodiazepines and certain types of other antidepressants (so called cyclic antidepressants like Imipramin, Clomipramin, Mirtazapine or Mianserin). The toxicity of these antidepressants can be masked by protective benzodiazepine effects (see also section 2 “Other medicines and XXXXX”).
Signs of a significant overdose with cyclic antidepressants include:
• dilation of the pupil, inability to urinate, dry mouth,
• severe or potentially life threatening conditions like agitation, breathing problems, convulsions, heart problems and coma.
Children
Children previously sedated with midazolam should be closely observed in intensive care units for at least 2 hours after administration of XXXXX. Repeated sedation or difficulty with breathing can occur. In case of sedation by other benzodiazepines, the monitoring must be adjusted according to their expected duration.
Children of 1 year and younger should only receive XXXXX if the risks have been carefully weighed against the advantages of the therapy.
Children should only receive XXXXX after deliberate sedation. There are insufficient data for any other indications. The same applies for children below the age of 1 year.
Other medicines and XXXXX
Please tell your doctor if you are taking or have recently taken any other medicines, including medicines obtained without a prescription.
XXXXX counteracts the effect of all medicines that act via the benzodiazepine receptor. This includes medicines that do not belong to the group of benzodiazepines, but that have the same active principle such as zopiclone (like Zimovane), triazolopyridazine and others.
Benzodiazepines can mask the toxic effects of certain psychotropic medicinal products (especially tricyclic antidrepressants like Imipramin, see also section 2 “Warnings and precautions”). When using XXXXX in cases of an accidental overdose it has to be taken into account that the toxic effects of such medicines taken concurrently may increase with the subsidence of the benzodiazepine effect.
Interaction with other central nervous system depressants or alcohol has not been observed.
Pregnancy and breast-feeding
If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
Because of insufficient experience during pregnancy, XXXXX should only be used if the advantage for you is higher than the potential risk for the unborn baby. The administration of XXXXX during pregnancy is not contraindicated in an emergency situation.
It is not known whether XXXXX is excreted in breast milk. Therefore you should not breastfeed for 24 hours after administration of XXXXX. The administration of XXXXX during lactation is not contraindicated in an emergency situation.
Driving and using machines
After receiving XXXXX for the reversal of the sedative effects of benzodiazepines you must not drive a car, operate machinery or engage in any other physically or mentally demanding activity for at least 24 hours since sedation may possibly recur.
XXXXX contains sodium
This medicinal product contains 3.7 mg sodium per ml (18.5 mg / 5 ml ampoule size or 37 mg / 10 ml ampoule size) solution for injection. To be taken into consideration by patients on a controlled sodium diet.
3. How to use XXXXX
XXXXX will be given by your anaesthetist or experienced physician. XXXXX is administered as intravenous injection (into a vein) or diluted as intravenous infusion (over a longer period).
XXXXX may be used at the same time as other resuscitative measures.
Newborn infants, infants and toddlers under the age of 1 year
There are insufficient data on the use of XXXXX in children under 1 year. Therefore XXXXX should only be administered in children under 1 year if the potential benefits to the patient outweigh the possible risk (see also section 2 “Warnings and precautions, Children”).
Patients with renal (kidney) or hepatic (liver) impairment
In patients with impaired liver function, the elimination of XXXXX may be delayed and therefore careful adjustment of dosage is recommended.
No dosage adjustments are required in patients with impaired kidney function.

4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody gets them.
The following side effects may be serious. If any of the following side effects occur, stop using this medicine and consult a doctor immediately:
Common (may affect up to 1 in 10 people):
● allergic reactions,
● abnormal rapid and deep respiration (hyperventilation),
● speech disorder.
Uncommon (may affect up to 1 in 100 people):
● slow or rapid heart rate, premature beat of your heart (extrasystole),
● difficulty in breathing,
● chest pain.
Not known (frequency cannot be estimated from the available data):
● convulsions (in patients suffering from epilepsy or severe liver insufficiency, mainly after long-term treatment with benzodiazepines or multiple medicinal products abuse).
Other side effects include:
Common (may affect up to 1 in 10 people):
● agitation (after rapid injection, not requiring treatment), strong emotional lability,
● having problems in initiating and maintaining sleep (insomnia), feeling sleepy (somnolence),
● dizziness, headache,
● involuntary trembling or quivering (tremor),
● dry mouth,
● sensations on the skin (e.g. cold, warmth, tingling, pressure, etc.) in the absence of stimulation (paresthesia),
● double vision, squinting (strabismus), increased production of tear fluid (increased lacrimation),
● sweating,
● low blood pressure, drop of blood pressure on transition from lying to standing (orthostatic hypotension),
● feeling sick (during anaesthesia)
● vomiting (during anaesthesia), hiccups,
● fatigue,
● pain at the injection site.
Uncommon (may affect up to 1 in 100 people):
● anxiety and fear (occurring after rapid injection, not requiring treatment),
● being aware of your heart rate (palpitations, occurring after rapid injection, not requiring treatment),
● abnormal hearing,
● cough, nasal congestion,
● reddening of the skin,
● shivering.
Not known (frequency cannot be estimated from the available data):
● panic attacks in patients that already showed panic reactions,
● transient increased blood pressure (on awaking),
● abnormal crying, agitation and aggressive reactions.
If you were treated for a long period with benzodiazepines XXXXX can induce withdrawal symptoms (frequency not known). The symptoms include: agitation, anxiety, emotional lability, confusion and abnormal sensory perception.
In general the undesirable effects in children do not differ much from that in adults. When XXXXX is used to wake up your child from sedation, abnormal crying, agitation and aggressive reactions might occur.
If you get any side effects, talk to your doctor or pharmacist. This includes any possible side effects not listed in this leaflet.
5. How to store XXXXX
Keep this medicine out of the sight and reach of children.
XXXXX must not be used after the expiry date which is stated on the label and carton. The expiry date refers to the last day of that month.
Do not store above 25°C
This medicinal product is for single use only.
Shelf life after first opening: the medicinal product should be used immediately.
Shelf life after dilution: 24 hours.
Chemical and physical in-use stability has been demonstrated for 24 hours at 25°C.
From a microbiological point of view, the product should be used immediately. If not used immediately, in-use storage times and conditions prior to use are the responsibility of the user and would normally not be longer than 24 hours at 2 to 8 °C, unless dilution has taken place in controlled and validated aseptic conditions.
The solution should be inspected visually prior to use. Do not use XXXXX if the solution is not clear, colourless and free from particles.
Any unused solution should be discarded in accordance with local requirements.
Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

1. Qué es XXXXX y para qué se utiliza
XXXXX es un agente antagonista (antídoto) para un grupo específico de medicamentos denominados benzodiacepinas. Las benzodiacepinas tienen propiedades sedantes, inductoras del sueño y de relajación muscular. Se utilizan para inducirle el sueño y tranquilizarle si tiene ansiedad. El XXXXX puede revertir de forma completa o parcial estos efectos.
XXXXX puede utilizarse
• en anestesia para despertarle después de una cirugía o ciertas pruebas diagnósticas
• si se le ha mantenido en condiciones de sedación en cuidados intensivos
XXXXX puede también ser empleado para el diagnóstico y tratamiento de las intoxicaciones o sobredosis con benzodiacepinas.
XXXXX se utiliza en niños mayores de 1 año para despertarles después de recibir benzodiacepinas para inducirles el sueño durante una intervención médica.
2. Qué necesita saber antes de empezar a usar XXXXX
No usar XXXXX
− Si es alérgico al XXXXX o a cualquiera de los demás componentes de este medicamento (incluidos en la sección 6).
− Si le han administrado benzodiacepinas para el control de una situación de riesgo vital (por ejemplo control de la presión en el cerebro o un ataque epiléptico grave)
Advertencias y precauciones
Se debe tener especial cuidado
− Si usted es epiléptico y ha recibido tratamiento con benzodiacepinas durante un largo periodo de tiempo, en este caso, la administración de XXXXX puede producir convulsiones.
− Si tiene daño cerebral grave (y/o presión inestable en su cerebro), puesto que el XXXXX puede ocasionar un aumento de presión en su cerebro.
− Si sufre alguna enfermedad hepática. Su médico ajustará cuidadosamente la administración de XXXXX.
− Si ha tenido crisis de angustia en el pasado, puesto que el XXXXX puede provocar nuevos ataques.
− Si está muy nervioso debido a su operación o si tiene un historial de ansiedad. Su médico ajustará cuidadosamente la administración de FXXXXX.
− Si se le ha tratado durante periodos largos con elevadas dosis de benzodiacepinas, puesto que se corre el riesgo de síntomas de abstinencia (los síntomas de abstinencia se detallan en la sección 4. “Posibles efectos adversos”).
− Si es adicto al alcohol o los medicamentos. En este caso usted tiene un riesgo mayor de tolerancia a la benzodiacepina y a la dependencia.
− Si tiene una enfermedad coronaria. Se debe informar a su médico para poder tenerle más tiempo bajo sedación.
Su estado de alerta y las constantes vitales (como la presión arterial, frecuencia cardíaca y la respiración) se controlarán durante un periodo de tiempo adecuado después de haber recibido XXXXX. Como la acción del XXXXX suele ser más corta que la de las benzodiacepinas, puede volver a presentarse sedación. Usted estará atentamente vigilado posiblemente en la unidad de cuidados intensivos hasta que los efectos de XXXXX hayan desaparecido por completo.
Si se le administra XXXXX al final de su operación para despertarle, no se le debe administrar hasta que los efectos de los relajantes musculares hayan desaparecido.
Su médico considerará el dolor postoperatorio antes de darle XXXXX.
Si no se despierta después de la administración de de XXXXX, debe considerarse otra razón para ello, puesto que el XXXXX revierte específicamente los efectos de las benzodiacepinas.
Su médico evitará la inyección rápida de XXXXX.Si usted ha recibido un tratamiento prolongado (crónico) con benzodiacepinas, la inyección rápida de elevadas dosis de XXXXX (más de 1 mg) puede producir síntomas de abstinencia.
XXXXX no se recomienda para el tratamiento de la dependencia a benzodiacepinas o de los síntomas de abstinencia de benzodiacepinas.
Su médico le administrará XXXXX sólo con especial precaución en caso de intoxicaciones mixtas con benzodiacepinas y ciertos tipos de antidepresivos (los llamados antidepresivos cíclicos como imipramina, clomipramina, mirtazapina o mianserina). La toxicidad de estos antidepresivos se puede enmascarar por los efectos protectores de la benzodiacepina (ver también la sección 2 “Otros medicamentos y el XXXXX”).Los signos de una sobredosis significativa con antidepresivos cíclicos incluyen:
− dilatación de las pupilas, incapacidad para orinar, boca seca,
− condiciones graves o potencialmente mortales como agitación, problemas respiratorios, convulsiones, problemas cardíacos y coma.
Niños
− Los niños que han sido previamente sedados con midazolám deben ser estrechamente observados en las unidades de cuidados intensivos durante al menos 2 horas después de la administración de XXXXX .
− En caso de sedación mediante otras diazepinas, la monitorización se debe ajustar de acuerdo con su duración esperada.

Otros medicamentos y XXXXX
Informe a su médico o farmacéutico si está utilizando o ha utilizado recientemente otros medicamentos, incluso los adquiridos sin receta médica.
El XXXXX contrarresta el efecto de todos los medicamentos que actúan a través del receptor de las benzodiacepinas. Esto incluye a los medicamentos que no pertenecen al grupo de las benzodiacepinas pero que tienen su mismo principio activo, como la zopiclona (como el Zimovane), triazolopiridazina y otros.

No se conoce si XXXXX se excreta en leche materna. No debe amamantar durante 24 horas después de la administración de XXXXX. La administración de XXXXX durante la lactación no está contraindicada en una situación de urgencia.
Conducción y uso de máquinas
Después de recibir XXXXX para corregir los efectos sedantes de las benzodiacepinas, no puede conducir, operar maquinaria o iniciar cualquier otra actividad que requiera esfuerzo físico o mental durante al menos 24 horas ya que puede volver a aparecer sedación.
XXXXX contiene sodio
Este medicamento contiene 3,7 mg de sodio por ml (18,5 mg/tamaño de ampolla de 5 ml o 37 mg/tamaño de ampolla de 10 ml) de solución inyectable, lo que debe ser tenido en consideración en pacientes con dietas pobres en sodio.
3. Cómo usar Flumazeni l
XXXXX debe administrarse por su anestesista o médico experimentado. XXXXX se administra como inyección intravenosa (en una vena) o diluido como perfusión intravenosa (durante un periodo prolongado).
XXXXX puede ser utilizado al mismo tiempo que otras medidas de reanimación.
Bebés recién nacidos, bebés y niños pequeños menores de 1 año
No hay datos suficientes sobre el empleo de XXXXX en niños menores de 1 año.
Por lo tanto, XXXXX sólo deberá administrarse en niños menores de 1 año si los beneficios potenciales para el paciente superan a los posibles riesgos (ver también la sección 2 “Advertencias y precauciones”).
Pacientes con disfunción renal (riñones) o hepática (hígado)
En pacientes con disfunción del hígado, puede retrasarse la eliminación de XXXXX y por ello se recomienda un ajuste cuidadoso de la dosis.
En pacientes con disfunción en los riñones no se requieren ajustes de la dosis.
Si tiene cualquier otra duda sobre el uso de este medicamento, pregunte a su médico o farmacéutico.

4. Posibles efectos adversos
Al igual que todos los medicamentos, este medicamento puede tener efectos adversos, aunque no todas las personas los sufran.
Los siguientes efectos adversos pueden ser graves. Si tiene lugar cualquiera de los siguientes efectos adversos, deje de usar este medicamento y consulte inmediatamente con un médico.
Frecuentes (pueden afectar a 1 de cada 10 personas):
− reacciones alérgicas
− respiración anormal rápida y profunda (hiperventilación)
− trastornos del habla
Poco frecuentes (pueden afectar hasta a 1 de cada 10 personas):
− ritmo cardíaco lento o rápido, latido prematuro de su corazón (extrasístole)
− dificultad al respirar
− dolor torácico
Frecuencia no conocida (no se puede estimar la frecuencia a partir de los datos disponibles):
− convulsiones (en pacientes que sufran epilepsia o insuficiencia hepática grave, sobre todo después del tratamiento a largo plazo con benzodiacepinas o el abuso de múltiples medicamentos.
Otros efectos adversos incluyen:
Frecuentes (pueden afectar a 1 de cada 10 personas):
− agitación (después de una inyección rápida, no requiere tratamiento), gran labilidad emocional,
problemas en el inicio y mantenimiento del sueño (insomnio), sensación de sueño (somnolencia),
− mareos, dolor de cabeza,
− movimientos temblorosos involuntarios o temblorosos (temblor),
− sequedad bucal,
− sensaciones cutáneas subjetivas (p. ej., frío, calor, hormigueo, presión, etc.) en ausencia de estimulación (parestesia),
− visión doble, bizqueo (estrabismo), producción incrementada de flujo lacrimal (aumento del lagrimeo)
− presión arterial baja, tensión sanguínea baja al cambiar de posición supina a erecta (hipotensión ortostática),
− malestar (durante la anestesia),
− vómitos (durante la anestesia), hipo,
− fatiga,
− dolor en la zona de inyección.
Poco frecuentes (pueden afectar hasta a 1 de cada 10 personas):
− ansiedad y miedo (después de una inyección rápida, no requiere tratamiento),
− consciencia de su ritmo cardíaco (palpitaciones, que se producen después de una inyección rápida, no requiere tratamiento),
− audición anormal,
− tos, congestión nasal
− enrojecimiento de la piel
− escalofríos
Frecuencia no conocida (no se puede estimar la frecuencia a partir de los datos disponibles):
− ataques de angustia en pacientes que ya han mostrado reacciones de angustia,
− aumento transitorio de la presión arterial (al despertarse),
− llanto anormal, agitación y reacciones agresivas.
Si usted es tratado durante periodos prolongados con benzodiacepinas, el XXXXX puede inducir síntomas de abstinencia (se desconoce la frecuencia). Los síntomas incluyen: agitación, ansiedad, debilidad emocional, confusión y percepción sensorial anormal.
En general, los efectos indeseables en niños no difieren mucho de los de los adultos. Cuando se utiliza XXXXX para despertar a su niño de la sedación, puede tener lugar lloro anormal, agitación y reacciones agresivas.
Si tiene algún efecto adverso hable con su médico o farmacéutico. Esto incluye cualquier posible efecto adverso no mencionado en este prospecto.
5. Conservación de XXXXX
Mantener este medicamento fuera de la vista y del alcance de los niños.
No use XXXXX después de la fecha de caducidad que aparece en la etiqueta y en la caja. La fecha de caducidad es el último día del mes que se indica.
No conservar a temperatura superior a 25°C.
Este medicamento es para un solo uso únicamente.
Periodo de validez después de la primera apertura del envase: el medicamento deberá usarse de inmediato.
Periodo de validez después de la dilución: 24 horas.
Se ha demostrado la estabilidad química y física en uso durante 24 horas a 25 ºC.
Desde un punto de vista microbiológico, el producto deberá administrarse inmediatamente. Si no se administra de inmediato, los tiempos de conservación en uso y las condiciones antes del uso son responsabilidad del usuario y, normalmente, no serían más prolongadas de 24 horas a una temperatura de 2 a 8 ºC, a menos que la dilución se haya producido en condiciones asépticas controladas y validadas.
Se debe inspeccionar visualmente la solución antes de utilizarla. No use XXXXX, si la solución no es transparente, incolora y libre de partículas.
Cualquier solución no usada debe eliminarse de acuerdo con los requerimientos locales.
No tire ningún medicamento por los desagües ni a la basura. Pregunte a su farmacéutico cómo tirar los medicamentos que no necesita. De esta forma ayudará a proteger el medio ambiente.

You have been diagnosed with B-precursor acute lymphoblastic leukemia (ALL). Depending on the state of your individual treatment you have achieved or you may achieve a disease state called “mini-mal residual disease” (MRD) in the future. The term “minimal residual disease” means that very small numbers of tumor cells have remained in the bone marrow after chemotherapy, which cannot be de-tected under the microscope by usual methods but can be detected by specific laboratory methods. Previous observations have shown that these remaining leukemia cells may proliferate again and lead to a relapse of the disease.
Therefore you may become eligible for a clinical research study in the near future. In this study the efficacy, safety and tolerability of the treatment with a new drug called Blinatumomab will be tested in patients with minimal residual disease (MRD) of B-precursor acute lymphoblastic leukemia (ALL). Blinatumomab (also known as MT103) is supposed to find and attack these remaining leukemia cells in your bone marrow. It is currently in clinical development and has not yet been approved for market-ing by any health authority. This clinical study is sponsored by Micromet AG, Staffelseestr. 2, 81477 Munich, Germany

It contains important information about the objectives and the course of this prescreening research that will help you to decide whether to take part. WHAT IS THE PURPOSE OF THIS PRESCREENING?
During the course of your treatment and thereafter, your bone marrow is analysed at regular time points to investigate the development and status of the disease as part of the clinical routine. While many hospital laboratories can analyse bone marrow by a common technique called flow-cytometry, only few laboratories in Europe are able to analyse bone marrow by a much more sensitive technique, the so-called RQ-PCR. We are asking for your consent to analyse your bone marrow on the presence of residual leukemic cells in one of these specialized laboratories and to store the cells for use in the above mentioned research later. Your doctor will inform you if a laboratory in your country is able to do the analysis or if it needs to be shipped to an external laboratory. The results will be made available to you and your doctor as soon as they are obtained.
The purpose of this prescreening is to identify as soon as possible whether you may become eligible to participate in the before mentioned clinical study for patients with minimal residual ALL by shipping, analysing and storing your bone marrow material from the time you were first diagnosed with ALL until you may have achieved the disease state of minimal residual disease and are considering to participate in the clinical trial.
DO I HAVE TO AGREE INTO THIS PRESCREENING?
Taking part in the prescreening research is your choice meaning that your participation is absolutely voluntary and requires your prior written consent. You are free to withdraw at any time, without giving a reason. This would not affect the care you receive at any time. If you agree to participate in the pre-screening you are still free to decide if you want to participate in the clinical study in the future. You will be asked and informed for the clinical study separately if you are diagnosed with minimal residual disease at a later point of time.
WHAT WILL HAPPEN TO ME IF I AGREE INTO THIS PRESCREENING?
If you agree to take part in this prescreening research by signing this written informed consent, a sam-ple of your bone marrow material obtained since the time point of primary diagnosis will be shipped to a specialized laboratory, where it will be analysed and stored for the purpose of identifying residual leukemic cells.
Your name, address and any other personal data will be removed and the sample will be coded. If you do not agree, the bone marrow material already taken during your routine treatment will remain archived where it currently is archived (usually at the department of pathology or another local labora-tory associated to the hospital where you are treated) and future samples will also remain to be ana-lysed where they are currently analysed.
What will happen if I change my mind?
If you do decide to take part you are still free to change your mind at any time. You have the right to withdraw your consent to store your bone marrow sample without giving a reason. If you do withdraw, it will not affect the treatment that you are receiving. You can withdraw your consent by contacting your doctor (contacting details at the end of this patient information)
The bone marrow stored in the central laboratory will be destroyed and a letter of confirmation will be sent to your doctor. If you change your mind a long time after the samples were taken, then some research may have already taken place on your samples. Your doctor (or the local laboratory) would not be able to recall samples and information once they have been used, but your doctor (or the la-boratory) would request the return of any unused samples for adequate disposal. This would ensure that no further research work will be undertaken on your bone marrow sample.
WHAT DO I HAVE TO DO WHILE PARTICIPATING IN THIS PRESCREENING?
You do not have to follow special advices that you would not receive when not participating in this prescreening. Nevertheless, make sure to follow any advice your doctor gives to you.

If you agree to participate in the prescreening you keep open the option for your possible participation in the clinical study in the future. The results of further research carried out using your bone marrow samples, and those of others, may also help in the future discovery of new or better diagnostic techniques.

Source text - English The common view of viruses, mostly true, is of tiny burglars that sneak into cells, grab the biosynthetic controls and compel the cell to make huge numbers of progeny that break out of the cell and keep the replication cycle going. Viruses are supposed to be diminutive even compared to cells that are just a micrometer (1,000 nanometers) in diameter. They are supposed to travel light, making do with just a few well-adapted genes.
In 1992, a new microorganism was isolated from a power-plant cooling tower in Bradford, England, where Timothy Robotham, a microbiologist at Leeds Public Health Laboratory, was seeking the causative agent of a local pneumonia outbreak. His search led to the warm waters of the cooling tower, a known reservoir for bacterial pathogens in the Legionella genus, which are the cause of the pneumonialike Legionnaire’s disease. Particles present in the sample were mistakenly identified as bacteria. Gram positive and visible under the microscope as pathogens within the particle-gobbling amoeba Acanthamoeba polyphaga, the entities surprisingly did not generate any product from the gene-amplifying polymerase chain reaction technique using universal bacterial primers.
Eleven years later, in 2003, the mystery organism received a new identity and a new name, Acanthamoeba polyphaga Mimivirus, for microbe-mimicking virus. Mimivirus is the largest virus ever discovered. Giant viruses had been known for a few years, many of them in a group termed nucleo-cytoplasmic large DNA viruses (NCLDVs). This group features several other virus families, including Poxviridae, which infects vertebrates (for example, smallpox virus) and invertebrates, the aquatic viruses Iridoviridae and Phycodnaviridae, and the vertebrate virus Asfarviridae. Giant viruses are considered to be ones with genomes larger than 300 kilobase pairs and with capsid diameters of about 200 nanometers or more.
Mimivirus is a giant among giant viruses, with a diameter of 750 nanometers. It possesses a genome, truly outsized by viral standards, of 1.2 million base pairs, coding an outlandish 1,018 genes. For comparison, the smallest free-living bacterium, Mycoplasma genitalium, is just 450 nanometers in diameter and possesses a genome half the size of that in mimivirus, while coding just 482 proteins. The record tiniest celular organism, Hodgkinia cicadicola, a parasite in cicadas that was described in 2009, has a genome of just 140,000 base pairs, coding a paltry 169 proteins. H. cicadicola is unable to live on its own, being entirely dependent on the lush environment of specialized cicada cells. Viruses are generally not considered living organisms (although for a consideration of their position in the phylogenetic tree of life, see the box on pages 308–09), yet mimivirus brings a bigger blueprint and more lumber to the replication process than the living H. cicadicola and many other bacteria.
Most giant viruses have only been discovered and characterized in the past few years. There are several reasons why these striking biological entities remained undetected for so long. Among the most consequential is that the classic tool for isolating virus particles is filtration through filters with pores of 200 nanometers. With viruses all but defined as replicating particles that occur in the filtrate of this treatment, giant viruses were undetected over generations of virology research. (Mimivirus disrupted this evasión tactic by being so large it was visible under a light microscope.) Standard plaquing procedures failed to report the presence of giant viruses because the large particles bogged down in the soft agar of the plaquing medium, disrupting diffusion and the formation of visible plaques. An additional explanation for the elusiveness of the largest viruses is that many infect protists, which have received far less research attention than plants and animals.
With the spotlight finally on them, the giant viruses are delivering striking lessons in viral physiology and ecology, not to mention challenging long-held assumptions about the shape of the phylogenetic tree of life.

Source text - English The inclusion of respiratory tract infections such as acute and chronic bronchitis, bacterial pneumonia, infected bronchiectasis, lung abscess and post-operative chest infections; kidney and urinary tract infections; obstetric and gynaecological infections such as pelvic inflammatory disease; infections of the skin and soft tissues; septicaemia; and meningitis are fully supported by the large number of studies in the published literature. Doses of 1 g or 2 g are utilised in the vast majority of studies in all indications. Dosing in earlier studies was often eight hourly but more recent studies have demonstrated the adequacy of twelve hourly dosing. In very severe, intractable infections such as meningitis higher and more frequent dosing is more usually implemented. This range of dosing is fully addressed in the SPC for Cefotaxime BIOCHEMIE.
Similarly, the efficacy of cefotaxime as a single 1 g dose in the treatment of gonorrhoea is confirmed by the clinical studies. Also the use of cefotaxime as prophylaxis of peri- or post-operative infections, has been extensively researched. In this indication, many studies have utilised cefotaxime pre-, peri-, and post-operatively, but more recent studies support the use of a single 1 g or 2 g pre-operative dose. Thus these indications are justifiably included in the SPC for Cefotaxime BIOCHEMIE, and the specific dosing regimens specified in the SPC are appropriate.

CT with contrast or MRI covering head and neck, lung and liver fields are required for all patients and will be collected for central review by an independent radiology review committee as further described in the study manual and a separate imaging charter. The tumors will be assessed following the revised RECIST Guidelines version 1.1 (Eisenhauer et al., 2009). Evaluation of tumor status will be conducted at baseline, at the end of week 6 on study and then every 6 weeks on and after study until disease progression, study termination, initiation of subsequent anticancer therapy, death, loss to follow-up or withdrawal of consent. Note, the scans should be performed on a calendar schedule (not to be affected by treatment delays or drug holidays).
Pharmacodynamic Endpoints
At some selected study sites, patients whose tumors are accessible to biopsy will be asked to undergo tumor biopsy (baseline and post therapy) to evaluate Ras pathway status, HPV status and to measure viral replication within the tumor. The timing of the second biopsy will vary, based on response, in order to optimise the data generated by this assessment.
Toxicity Requiring Dose Delay/Reduction
Dose delay/reduction is required if any one or more of the following events is determined to be possibly, probably or definitely related to paclitaxel and/or carboplatin and/or REOLYSIN® (as determined by the investigator). If treatment is resumed, the doses of therapy must be reduced even if the toxicity resolved (see Section 6.7 below).
• ANC < 0.5 x 109/L lasting for > 7 days,
• ANC < 0.1 x 109/L lasting for > 3 days, or
• ANC < 0.5 x 10 9/L with fever (>100.5º F or 38.1ºC).
• Platelet count < 25 x 109/L.
• Grade >2 cardiotoxicity, persistent Grade 2 neurotoxicity. Discontinue if > Grade 3 cardiotoxocity or neurotoxicity occurs.
• Any other drug-related non-hematological Grade 3/4 toxicity.
Management of Dose Reduction
Hematologic Toxicity
Patients experiencing any toxicity as described in Section 6.6 in any cycle will have their REOLYSIN® and/or paclitaxel and carboplatin treatments held until toxicity resolves to baseline or Grade 1. Upon resolution, REOLYSIN®, paclitaxel and carboplatin therapy may recommence at a lower dose level. Dose reduction of paclitaxel and/or carboplatin will be carried out according to the following guidelines:
• Reduction of carboplatin to AUC 4 and paclitaxel to 135 mg/m2 after:
ANC < 0.5 x 109/L lasting for > 7 days, or
ANC < 0.1 x 109/L lasting for > 3 days, or
ANC < 0.5 x 109/L with fever (>100.5º F or 38.1ºC).

Toxicidad que Requiera Retardo/Reducción de Dosis
El retraso/reducción de dosis es necesaria si uno o más de los siguientes eventos se determina como posible, probable o definitivamente relacionado con el paclitaxel y/o con el carboplatino y/o el REOLYSIN® (según lo determinado por el Investigador). Si el tratamiento se reanuda, la dosis de la terapia debe ser reducida, aunque la toxicidad se haya resuelto (véase Sección 6.7 más adelante).

Gestión de la Reducción de la Dosis
Toxicidad Hematológica
Los pacientes que experimenten una toxicidad en cualquier ciclo tal, y como se describe en la Sección 6.6, tendrán suspendido su tratamiento con REOLYSIN® y/o paclitaxel y carboplatino hasta que la toxicidad vuelva a la línea de base o sea de Grado 1. Tras la resolución, la terapia con REOLYSIN®, paclitaxel y carboplatino puede reanudarse con una dosis más baja. La reducción de la dosis de paclitaxel y/o carboplatino se llevará a cabo de acuerdo con las siguientes pautas:

There were 5 cases of breast cancer per 4,591 person-years of follow-up in the XXXXX 20 mg group (1.09 per 1,000), 7 cases of breast cancer per 4,526 person-years of follow-up in the XXXXX 60 mg group (1.55 per 1,000), and 8 cases of breast cancer per 4,604 person-years of follow-up in the placebo group (1.74 per 1,000). Source text - English Biomechanical Prerequisites
The biomechanical examination of osteosynthesis procedures requires the calcula-tion of numerous physical quantities. To ensure a basic mechanical knowledge for the subsequent examination of selected applications some common mechanical pa-rameters and units will be introduced briefly in this first chapter.
The aim of this module is to present computer methods for the stability analysis of modern osteosynthesis procedures. Among other things, it is essential to determine the strength of the bone and the implant as well as the strength of the combined structure of bone and implant. Analyzing the strength of a structure involves information about acting loads and the consideration of the mechanical load capacity.
Loads in human body:
Loads act permanently on the human body, and are distinguished into two different categories: those with an origin outside of the body (e.g. gravity, inertia, contact forces) and those which are initiated inside the body (e.g. muscle forces, interstitial pressure).
Forces in human body – physical description:
Loads which are acting on an object through application from outside (e.g. interaction with other objects) are described by forces. The force is a directed (vectored) mechanical quantity and hence includes a point of application, a direction and a corresponding magnitude which specifies the extent of the load. Forces constitute the cause for movement and deformation of bodies and are quantified by the unit Newton [N].
At the earth's surface the mass of one kilogram induces a force of about 10N. This approximation is suggestive of the magnitude of considered loads.
Example: Force acting on helical spring (see Figure 1)
The spring is compressed to about the length d (effect) by the acting force F (cause).
The change in length is dependent on the magnitude of the force and the tendency of the spring to counteract the force (so-called stiffness).
The moment of force:
In addition to the description of loads through forces there exists the definition of moment of force or torque (“rotational force”) in classical mechanics. A moment causes rotational movement and deformation through bending or torsion. The mo-ment arises from a force through its eccentric loading direction with reference to a center of rotation. It is immaterial whether the rotation about this center is free or constrained.
The moment as a mechanical quantity results from the relationship “force times lever arm”. The lever arm describes the shortest distance between center of rotation and effective direction of the load. According to its calculation the physical unit of the moment yields newton meter [Nm]. The moment is also a directed (vectored) quantity and is characterized by its sense of rotation (left / right).
Example: Force acts with perpendicular distance e to the effective direction on a helical spring (see Figure 2)
The spring is shortened (length d) due to the loading by force F. Additionally, the spring is rotated at its end (angle α) caused by the eccentric point of application of force F. This condition is mechanical equivalent to a centered load F and a moment M orientated about the point of application of the force.
Balance of forces and moments:
For a system at rest (static system) the balance of forces and moments has always to be satisfied. Otherwise the system will accelerate.
Example: Scale (see Figure 3)
For the scale to stay at rest, the balance of moments must exist about the center of rotation (sum of all moments about a center of rotation equals zero or “counterclock-wise = clockwise”, respectively):
Moreover, the balanced scale passes a force A to the ground. The force A follows from the balance of forces (sum of all forces equals zero; mass of scale neglected):
Biomechanical example – contact force at hip joint:
Using classical mechanics, loadings in human body can be approximated roughly. If we consider the load situation at the hip joint during single leg stance with high sim-plification (Figure 4), the resulting force K at the hip joint can be approximated with the help of the equilibrium of moments about the center of rotation B. A stable single leg stance results if the moment which originates from the muscle force (generated by the muscle force M of the abductors) balances the moment due to the force G of the body mass. From the ratio of the lever arms AB to BC, an approximation results for the muscle force of about 2.5 times the body weight. Furthermore, this yields a value for the contact force K at the hip joint of about 3.5 times the body weight.
Through the action of numerous muscles, loads in many regions of the human body turn out to be substantial higher than the body weight itself. For specific joints of the human body these loads can be validated through in vivo measurements (e.g. with instrumented implants) [1, 2].
Internal loads in human body:
So far, only external loads on an object (or here on a specific tissue) have been con-sidered. Due to the external loads, the mechanical situation inside of an object is in-fluenced as well. Throughout the following considerations, it is assumed that all de-formations are elastic and thus reversible. From this follows that the deformed sys-tem returns entirely to its initial state after unloading.
The previously described quantities force and moment are introduced punctual from outside as summed quantities (so-called resultants) into a mechanical system. This simplification is not useful in consideration of the internal condition of a body, be-cause it is subject to high local variations dependent on material properties and geo-metrical aspects. For this reason, quantities which are related to geometrical dimen-sions (e.g. to lines, areas or volumes) and describe the mechanical condition locally are introduced (Figure 5).
Stress:
The counterpart to the macroscopic force quantity represents the so-called mechani-cal stress. Stress is defined as force per area and is further subdivided according to its mode of action. Stresses which result from tension or compression are called normal stresses, while shear stresses originate from loading in transverse direction.
Stress characterizes the internal loading of a body and can be understood as distri-buted internal force. It is common to indicate stresses with the unit Newton per square millimeter [N/mm2] or alternatively in Pascal [Pa] (1 [N/mm2] = 1 [MPa] ).
Strain:
To be able to determine not only the load situation within an object but also its de-formation, the quantity of strain is introduced into the mechanical framework.
As an example, consider a bar under tensional load. The relation of change in length to initial length describes the strain in the bar. A bar with initial length 1m, which elongates about 0.2mm under tension, is experienced to strain in longitudinal direction of 0.2mm / 1m = 0.02% according to previous definition (Figure 6). Strains are dimensionless by nature but it is common to specify strains in percent or so-called Microstrain [με] (1με = 10-6m/m = 1μm/m).
Material law:
The previously described quantities stress and strain are linked through the material law. Thus the extent of a deformation for a given load is dependent on the constitu-tion of the considered material. Conversely, the internal loading for a specific defor-mation is determined by the material behavior. This property of the material to coun-teract the introduced force or deformation is generally known as material or intrinsic stiffness.
Using the simple material model of linear elasticity, the stresses and corresponding strains are linearly dependent (e.g.: twice the load results in twice the deformation). This relationship is valid for many materials in the range of small strains. The so-called elasticity modulus (Young's modulus) denotes the material stiffness. The ma-terial behavior for linear elasticity is expressed through Hooke's law:
The unit of the Young's modulus is specified analog to the unit of the stress in New-ton per square millimeter [N/mm2] or Pascal [Pa].
Hooke's law can be demonstrated with the help of a short example. Consider two bars from different materials under tension load (Figure 7 left). As an example, one bar is made of steel with Young's modulus ESteel = 210.000 MPa while the other bar consists of a surgical titanium alloy with ETitanium = 113.800 MPa. The tension load increases gradually from zero during experiment.
If the load is plotted against the evolving deformation for both bars, a corresponding stress-strain-diagram shows the depicted characteristics (Figure 7 right). The curves indicate that the strain at similar stress level is considerably lower for the steel bar with higher material stiffness. Accordingly, the stresses in the titanium bar at similar deformation are much lower due to lower material stiffness. The slope of the curves in the stress-strain-diagram indicates the Young's modulus of the corresponding material.
Structural stiffness:
The above expressed material stiffness has to be clearly separated from the so-called structural stiffness. The last is frequently taken as reference quantity in studies which address biomechanical parameters in the fields of orthopedics and trauma surgery. Dependent on the type of loading, the structural stiffness is distinguished in axial, bending and torsional stiffness. In addition to material properties, the structural stiffness incorporates geometrical effects which originate form properties of the loaded cross section of the considered structure. For example, a bar under tension load with higher cross sectional area requires a higher load to yield the same deformation as a corresponding bar with lower cross sectional area but the same material properties. Especially in case of bending or torsional loading, not only the cross sectional area but also the shape of the cross section is relevant for the evolving structural stiffness. In many real structures, such as bone, the cross section is subject to high and dramatic changes in longitudinal direction. As a result, the structural stiffness of structures is generally not assessed by analytical procedures. However, testing machines provide the possibility to determine structural stiffness with high accuracy by recording the introduced force or moment and the corresponding change in length or torsional angle, respectively (Figure 8).
Fracture gap movement:
The fracture gap movement or interfragmentary movement is known to be closely linked to healing success of bony fractures. The interfragmentary movement de-scribes the relative motion between two fragments, where again it is distinguished between axial, transverse and rotational movement according to its direction.
It has frequently been stated in past studies that shear movement negatively influ-ences healing and promotes pseudarthrosis, while slight axial movement acts as a stimulus for the healing process [3, 4]. Consequently, this parameter possesses high relevance with respect to fracture healing and is frequently used to evaluate different treatment procedures.
Simulation in biomechanics – a modern tool for clinical and theoretical research:
In addition to experimental testing in biomechanical examinations, computer assisted methods have become increasingly important. The possibility to repeat tests with virtual tissues arbitrarily in different variations provides increasing interest in numerical simulation in the fields of orthopedics and trauma surgery. Using the so-called Finite Element Method, complex mechanical situations of the musculoskeletal system can be modeled and quantified. The main advantage of the virtual over the real experiment is that the computer model yields the whole mechanical situation of an object under examination after solving the load case. In contrast, data in real experiments is limited to a comparably low number of discrete values which provide information about forces, strains or displacements in previously determined locations, dependent on the type of measurement.
The main disadvantage in numerical simulation is that the accuracy strongly depends on the modeling itself and on various numerical simplifications. A thorough revision of the computer model often involves high effort and is only possible in certain cases.
In the future, such models should provide the possibility to assist the physician in preoperative planning with the decision for a specific fixation procedure in clinical daily routine. Especially in cases of complex fracture situations, it is usually not obvious which treatment yields an adequate stability due to a huge variety of implant types and variations.
Apart from using simulation models in clinics, they can be efficiently integrated in the design optimization and redevelopment process of modern implants. The capability of numerical simulation is that it offers the possibility to examine numerous design variants at comparable low costs. Thus, high expenditures for large series of prototypes can be reduced. Moreover, a combination of Finite Element models with numerical optimization procedures offers the possibility to find ideal configurations also with parameters, which were not specified explicitly in advance (e.g. geometrical aspects).
The Finite Element Method – Principle:
The mechanical description of complex structures by analytical calculus is usually possible only with a high level of abstraction. The application of the Finite Element Method (FEM) permits to extend the amount of details under consideration enorm-ously. Thus, the modeling of a high level of geometrical details, complex material de-scriptions, as well as geometrically nonlinear behavior (e.g. contact), is common practice.
In principle, the posed problem becomes solvable by dividing the whole structure in numerous sub-domains (“finite elements”), on which the governing equations can be expressed (approximatively). In defining interactions between neighboring elements, the mechanical behavior of the whole structure is captured with regard to the external loads. This is done in compliance with kinematic conditions as well as under consideration of the characteristics of involved materials.

Para asegurar un conocimiento básico de mecánica para el posterior examen de las candidaturas seleccionadas, en este primer capítulo se introducen de forma breve algunos parámetros y unidades comunes de mecánica.
El propósito de este modulo es presentar los modelos informáticos para el análisis de la estabilidad de las técnicas modernas de osteosíntesis. Entre otras cosas, es esencial para determinar la resistencia del hueso y del implante, así como también, la resistencia de la estructura combinada del hueso y el implante. El resorte se comprime aproximadamente en una longitud d (efecto) por efecto de la fuerza F (causa) aplicada. La variación de la longitud es dependiente de la intensi-dad de de la fuerza y de la tendencia del resorte para contrarrestar la fuerza (la lla-mada rigidez).
El momento de la fuerza:
Además de la descripción de las cargas a través de las fuerzas, existe la definición del momento de una fuerza o par (“fuerza de rotación”) en la mecánica clásica. Un momento provoca el movimiento de rotación y la deformación a través de la flexión o de la torsión. El momento surge de una fuerza a través de su dirección de carga ex-céntrica con referencia a un centro de rotación. Si la rotación de este centro es libre o constreñida es irrelevante.
El momento, como magnitud mecánica, es consecuencia de la relación “el brazo de palanca multiplica la fuerza”. El brazo de palanca describe la distancia más corta entre el centro de rotación y la dirección efectiva de la carga. De acuerdo con el cálculo, la unidad física del momento es newton metro [Nm]. El momento también es una unidad dirigida (vectorial) y se caracteriza por su sentido de giro (izquierda/derecha).
Ejemplo: Fuerza que actúa perpendicularmente a una distancia e de la dirección efectiva sobre un resorte en espiral (ver Figura 2)
El resorte se acorta (longitud d) debido a la carga de la fuerza F. Además, el resorte gira en su extremo (ángulo α) debido al punto de aplicación excéntrico de la fuerza F. Esta condición es mecánicamente equivalente a una carga centrada F y un mo-mento M orientado sobre el punto de aplicación de la fuerza.
Balance de fuerzas y momentos:
Para un sistema en reposo (sistema estático) siempre debe satisfacerse el equilibrio de fuerzas y momentos. De lo contrario, el sistema se acelera.
Ejemplo: Balanza (ver Figura 3)
Para que una balanza esté en reposo, debe existir un equilibrio de los momentos alrededor del centro de rotación (la suma de todos los momentos alrededor del centro de rotación debe ser igual a cero o “sentido antihorario = sentido horario”, respectivamente):
Por otra parte, la balanza equilibrada pasa una fuerza A al suelo. La fuerza A resulta del equilibrio de fuerzas (la suma de todas las fuerzas es igual a cero; la masa de la balanza es despreciable):
Ejemplo biomecánico – fuerza de contacto en la articulación coxofemoral
Las cargas en el cuerpo humano se pueden aproximar de forma somera utilizando la mecánica clásica. Si tenemos en cuenta, con una gran simplificación, la situa-ción de carga en la articulación coxofemoral durante la postura sobre una sola pierna (Figura 4), se puede aproximar la fuerza K resultante en la articulación coxo-femoral con la ayuda del equilibrio de momentos alrededor del centro de rotación B. La postura sobre una sola pierna es estable si el momento originado de la fuerza muscular (generado por la fuerza muscular M de los abductores) equilibra el momento debido a la fuerza G de la masa corporal. De la relación de los brazos de palanca AB con BC, resulta una aproximación para la fuerza muscular de unos 2.5 veces el peso del cuerpo. Además, esto produce un valor para la fuerza de contacto K en la articulación coxofemoral de unas 3.5 veces el peso del cuerpo.
A través de la acción de numerosos músculos, las cargas en muchas regiones del cuerpo humano resultan ser sustancialmente mayores que el propio peso del cuer-po. Para articulaciones específicas del cuerpo humano estas cargas pueden ser va-lidadas a través de mediciones in vivo (por ejemplo, con los implantes instrumenta-dos) [1, 2].

Cargas internas en el cuerpo humano:
Hasta ahora, solamente se han considerado cargas externas sobre un objeto (o aquí sobre un tejido específico). Debido a las cargas externas, también se ve influenciada la situación mecánica en el interior de un objeto. A lo largo de las siguientes consideraciones, se asume que todas las deformaciones son elásticas, y por lo tanto, reversibles. De esto se desprende que el sistema deformado regresa por completo a su estado inicial después de la descarga.
Las magnitudes de fuerza y los momentos descritos previamente se introducen de forma puntual desde el exterior como magnitudes resumidas (las llamadas resultan-tes) dentro de un sistema mecánico. Esta simplificación no es útil si se considera la condición interna de un cuerpo, puesto que está sujeto a grandes variaciones locales que dependen de las propiedades del material y de aspectos geométricos. Por este motivo, se introducen las magnitudes que se relacionan con dimensiones geométricas (por ejemplo, líneas, áreas o volúmenes) y que describen a nivel local la condición mecánica (Figura 5).

Presión:
En contrapartida a la magnitud macroscópica de la fuerza se representa la llamada presión mecánica. La presión se define como fuerza por área y se subdivide según su modo de acción. Las presiones que resultan de la tensión o de la compresión se llaman presiones normales, mientras que las presiones de cizalla se originan a partir de una carga en dirección transversal. La presión caracteriza la carga interna de un cuerpo y se puede entender como fuerza interna distribuida. Es común indicar la presión con la unidad Newton por milímetro cuadrado [N/mm2] o alternativamente en Pascal [Pa] (1 [N/mm2] = 1 [MPa]).
Esfuerzo:
Para ser capaces de determinar, no sólo la situación de carga dentro de un objeto, sino también su deformación, se introduce la magnitud esfuerzo en el marco de la mecánica.
Como ejemplo, considérese una barra con carga de tensión. La relación de cambio en la longitud respecto a la longitud inicial describe el esfuerzo de la barra. Una ba-rra de una longitud inicial de 1m que se alarga unos 0.2mm debido a la tensión, ex-perimenta un esfuerzo en dirección longitudinal de 0.2mm/1m = 0.02% según la de-finición anterior (Figura 6). Los esfuerzos son adimensionales por naturaleza, pero es habitual especificar los esfuerzos en porcentaje o con el llamada Microesfuerzo [με] (1με = 10-6m/m = 1μm/m).
Ley de la Materia:
Las magnitudes de presión y esfuerzo descritas anteriormente están relacionadas a través de la ley de la materia. Así, el grado de deformación para una determinada carga es dependiente de la constitución del material considerado. Por el contrario, la carga interna para una deformación determinada, se mide por el comportamiento del material. Esta propiedad del material para contrarrestar la fuerza o la deformación introducidas se conoce generalmente como rigidez material o rigidez intrínseca.
Utilizando el modelo material simple de elasticidad lineal, la presión y los correspon-dientes esfuerzos son linealmente dependientes (por ejemplo: el doble de carga da lugar al doble de esfuerzo). Esta relación es válida para muchos materiales en un rango de esfuerzos pequeños. El llamado módulo de elasticidad (módulo de Young) indica la rigidez del material. El comportamiento de la materia en elasticidad lineal se expresa a través de la ley de Hooke.
Las unidades del módulo de Young se especifican de forma análoga a las unidades de la presión, en Newton por milímetro cuadrado [N/mm2] o Pascal [Pa].
La ley de Hook se puede demostrar con la ayuda de un pequeño ejemplo. Conside-remos dos barras de materiales diferentes bajo una carga de tensión (Figura 7 iz-quierda). A modo de ejemplo, una barra está hecha de acero, con un módulo de Young de EAcero = 210.000 MPa, mientras que la otra barra está formada por una aleación de titanio quirúrgico, con una ETitanio = 113.800 MPa. La carga de la tensión aumenta gradualmente a partir de cero durante el experimento.

El diagrama presión-esfuerzo muestra las características representadas (Figura 7 derecha) si se traza la carga frente a la evolución de la deformación para ambas ba-rras. Las curvas indican que el esfuerzo, a niveles similares de presión, es conside-rablemente menor para la barra de acero, con una mayor rigidez del material. En consecuencia, las presiones en la barra de titanio a una deformación similar son mucho menores debido a la menor rigidez del material. La pendiente de las curvas en del diagrama presión-esfuerzo indica el módulo de Young del material correspondiente.
Rigidez estructural:
La rigidez del material expresada anteriormente debe diferenciarse claramente de la llamada rigidez estructural. Esta última se toma con frecuencia como la magnitud de referencia en estudios referidos a los parámetros biomecánicos en el campo de la ortopedia y traumatología. Dependiendo del tipo de carga, en la rigidez estructural se distinguen la rigidez axial, la rigidez de flexión y la rigidez de torsión. Además de las propiedades del material, la rigidez estructural incorpora aspectos geométricos que se originan a partir de la carga de la sección transversal. Por ejemplo, una barra bajo carga de tensión con una mayor sección transversal requiere una carga mayor para producir la misma deformación que la correspondiente barra, con las mismas propiedades del material, pero con una sección transversal menor. Especialmente, para la evolución de la rigidez estructural en el caso de la carga de flexión o de torsión, es relevante no sólo el área de la sección transversal, sino también la forma de la sección transversal. En muchas estructuras reales, como el hueso, la sección transversal está sujeta a grandes y dramáticos cambios en dirección longitudinal. Como resultado, por lo general, no se evalúa la rigidez estructural de las estructuras por técnicas analíticas. Sin embargo, los aparatos de ensayo ofrecen la posibilidad de determinar la rigidez estructural con gran precisión mediante el registro de la fuerza introducida o del momento y el correspondiente cambio en la longitud o el ángulo de torsión, respectivamente (Figura 8).
Movimiento de la fisura de la fractura:
Se sabe que el movimiento de la fisura de la fractura o el movimiento interfragmenta-rio está estrechamente ligado al éxito de la consolidación de las fracturas óseas. El movimiento interfragmentario describe el movimiento relativo entre los dos fragmen-tos, donde de nuevo, de acuerdo con su dirección, se distingue entre el movimiento axial, transversal y de rotación.
En estudios anteriores, frecuentemente se exponía que el movimiento de cizalla in-fluye negativamente en la consolidación y facilita la pseudoartropatía, mientras que el movimiento axial leve actúa como un estímulo para el proceso de consolidación [3, 4]. Por lo tanto, este parámetro tiene una gran importancia en la consolidación de las fracturas y se utiliza con frecuencia para evaluar las diferentes técnicas de trata-miento.
Simulación en biomecánica – una herramienta moderna para la investigación clínica y teórica
Además de las pruebas experimentales en los exámenes de biomecánica, los méto-dos asistidos por ordenador han tomado una importancia creciente. La simulación numérica en el campo de la cirugía ortopédica y traumatológica ofrece un interés creciente debido a la posibilidad de repetir de manera arbitraria las pruebas con teji-dos virtuales con diferentes variaciones. Mediante el empleo del llamado Método de Elementos Finitos, se pueden modelar y cuantificar situaciones mecánicas comple-jas del sistema locomotor. La principal ventaja de los experimentos virtuales respecto a los reales es que, después de resolver el caso de carga, el modelo informático reproduce toda la situación mecánica del objeto en estudio. Por el contrario, los da-tos de los experimentos reales se limitan a un número comparativamente reducido de valores discretos que proporcionan, dependiendo del tipo de medición, informa-ción sobre fuerzas, esfuerzos o desplazamientos en lugares previamente determinados.
La principal desventaja de la simulación numérica es que la precisión depende en gran medida del propio modelado y en varias simplificaciones numéricas. Una revi-sión minuciosa del modelo informático implica, a menudo, un gran esfuerzo y sólo es posible en ciertos casos.
En el futuro, tales modelos deben ofrecer, en la rutina diaria clínica, la posibilidad de ayudar al médico durante la planificación preoperatoria en la toma de decisión de una técnica específica de fijación. Especialmente en casos de situaciones de fracturas complejas, normalmente no es evidente qué tratamiento ofrece una estabilidad adecuada, debido a una gran variedad de tipos de implantes y variaciones.
Aparte de utilizar modelos de simulación en clínica, éstos pueden ser integrados de forma eficiente en la optimización del diseño y en el proceso de reconstrucción de los implantes modernos. La habilidad de la simulación numérica es que ofrece la posibilidad de examinar numerosas variantes en el diseño con costes comparativamente bajos. Por lo tanto, se pueden reducir los elevados gastos para largas series de prototipos. Además, la combinación de modelos de Elemento Finito con unas técnicas de optimización numérica ofrece la posibilidad de encontrar las configuraciones ideales, también con los parámetros que no se han especificado explícitamente por adelantado (por ejemplo, los aspectos geométricos).
El Método de Elemento Finito – Principio:
La descripción mecánica de las estructuras complejas mediante el cálculo analítico es, en general, posible sólo con un nivel alto de abstracción. La aplicación del Méto-do de Elemenos Finitos (MEF) permite ampliar enormemente la cantidad de detalles considerados. Por lo tanto, es una práctica habitual el modelado con un alto nivel de detalles geométricos, descripciones de materiales complejos, así como el compor-tamiento geométricamente no lineal (por ejemplo, el contacto).
En principio, el problema planteado se puede resolver mediante la división de toda la estructura en varios subdominios (“elementos finitos”), en los que las ecuaciones que los gobiernan se pueden expresar (de forma aproximada). En la definición de las interacciones entre elementos vecinos, se capta el comportamiento mecánico de toda la estructura en lo que respecta a las cargas externas. Esto se hace en cumpli-miento de las condiciones cinemáticas, así como bajo la consideración de las carac-terísticas de los materiales implicados.

PHARMACOLOGY
Vayacog™ is a Phosphatidylserine-omega 3 compound, DHA enriched.
• Mechanism of Action
Administration of phosphatidylserine (PS) enriched with omega 3 fatty acids was found to significantly increase DHA level in rats brain [1]. DHA brain and plasma levels are suggested to be positively associated with cognitive performance [2,3].
While the exact mechanism by which Vayacog™ exerts its effects is not fully understood, PS in the mammalian nervous system, which is characterized by its substantial levels of omega-3 fatty acids, has been implicated in numerous membrane related functions, such as maintaining the integrity of cell membranes, cell excitability, cell-to-cell recognition and communication [4]. PS has been found to regulate key proteins in neuronal membranes, including sodium/calcium ATPase [5] and protein kinase C [6] which undertake crucial functions in diverse signal transduction pathways. Similarly, PS interacts with Raf-1 protein kinase to promote a cascade of reactions that are believed to be involved in cell survival [7]. Additionally, PS has been found to influence neurotransmitter activity, such as the release of acetylcholine, dopamine and noradrenaline [8, 9] and to increase brain glucose levels.
• Absorption and Metabolism
Following dietary ingestion of phospholipids, pancreatic digestive enzymes cleave specific fatty acids leading to the formation of lysophospholipids that are absorbed by the mucosal cells of the intestine and could be reacylated into phospholipid [10]. The fatty acids released can be further used for triglyceride synthesis. Because of the high activity of decarboxylases in the mucosal cells, the majority of the PS is converted into other phospholipids, primarily to phosphatidylethanolamine [11]. The reacylated PS, phosphatidylethanolamine and other phospholipids enter the lymph and circulation, and are redistributed.
• Drug Interactions
PS can potentially interact with some anticholinergic and cholinergic medications. It is recommended to consult with a physician about Vayacog™ interactions that may apply to specific medical conditions.
• Toxicity
Vayacog™, similar to phosphatidylserine extracted from bovine cortex (BC-PS), contains saturated and monounsaturated fatty acids as well as DHA. The safety profile of BC-PS was determined in several non-clinical studies. Repeat-dose safety studies in rats and dogs show that oral administration of BC-PS at doses up to 1000 mg/kg/day for up to 6 months was without any significant adverse effects of toxicological concern [12].
The results of teratogenicity studies in rats at doses up to 200 mg/kg/day and in rabbits at doses up to 450 mg/kg/day showed that oral administration of PS did not affect embryonic and fetal development [12].
The mutagenic potential of BC-PS was investigated in several cell types and revealed no significant findings. In a micronucleus test, BC-PS was administered to mice at total dosages of 30, 150 and 300 mg/kg in two equal doses separated by 24-hours. The results of the study did not reveal any evidence of mutagenic potential or bone marrow toxicity [12].
CLINICAL EXPERIENCE
Vayacog™ was evaluated in a randomized, double-blind, placebo-controlled study of 15 weeks followed by an open label extension of additional 15 weeks. In the double-blind phase, 157 men and women (aged 50-90 years) with memory complaints were randomly assigned to receive Vayacog™ or placebo (three capsules a day)[13]. During the open-label extension, all participants consumed one capsule a day of Vayacog™.
Cognitive performance was evaluated using Rey Auditory Verbal Learning Test (Rey-AVLT) at baseline and endpoint (week 15) of the double-blind phase and using a computerized cognitive assessment tool at baseline (week 15) and endpoint (week 30) of the open-label phase.
Safety assessment was evaluated by clinical laboratory assessments including biochemical and hematological parameters at baseline and endpoint (week 15) of the double-blind phase and by adverse events recording, physical examination and measurement of vital signs and weight at baseline, week 7 and endpoint (week 15) of the double-blind phase and at the end of the open-label extension (week 30).

FARMACOLOGÍA
Vayacog™ es un compuesto de Fosfatidilserina-omega 3 enriquecido con DHA.
• Mecanismo de Acción
La administración de fosfatidilserina (PS) enriquecida con ácidos grasos omega 3 se ha visto que aumenta significativamente los niveles de DHA en cerebro de ratas [1]. Se ha propuesto que los niveles de DHA en cerebro y plasma están asociados positivamente con el rendimiento cognitivo [2, 3].
Aunque el mecanismo mediante el cual VayacogTM ejerce sus efectos no se entiende completamente; la PS, en el sistema nervioso de mamíferos, que se caracteriza por sus niveles importantes de ácidos grasos omega-3, se la ha visto implicada en numerosas funciones relacionadas con la membrana, tales como el mantenimiento de la integridad de las membranas celulares, la excitabilidad celular, el reconocimiento célula-célula y la comunicación [4]. Se ha visto que la PS regula proteínas clave en las membranas neuronales, incluyendo la ATPasa sodio/calcio [5] y la proteína quinasa C [6], que lleva a cabo funciones esenciales en diversas vías de transducción de señales. Del mismo modo, la PS interactúa con la proteína quinasa Raf-1, para promover una cascada de reacciones de se creen involucradas en la supervivencia celular [7]. Además, se ha visto que la PS influye en la actividad de los neurotransmisores, como la liberación de acetilcolina, dopamina y noradrenalina [8, 9] y en el aumento de los niveles de glucosa en el cerebro.
• Absorción y Metabolismo
Después de la ingestión de los fosfolípidos en la dieta, las enzimas digestivas pancreáticas rompen los ácidos grasos, dando lugar a la formación de lisofosfolípidos que son absorbidos por las células de la mucosa del intestino y pueden ser reciclados en fosfolípidos [10]. Los ácidos grasos liberados pueden ser, posteriormente, utilizados para la síntesis de triglicéridos. Debido a la alta actividad descarboxilasa en las células de la mucosa, la mayoría de la PS es convertida en otros fosfolípidos, principalmente fosfatidiletanolamina [11]. La PS reciclada, la fosfatidiletanolamina y otros fosfolípidos entran en la linfa y en la circulación y son redistribuidos.
• Interacciones con otros medicamentos
La PS puede interaccionar potencialmente con medicamentos colinérgicos y anticolinérgicos. Se recomienda consultar a un facultativo sobre las interacciones con Vayacog™ que puedan aplicarse a determinados problemas de salud.
• Toxicidad
Vayacog™, similar a la fosfatidilserina extraída de la corteza bovina (BC-PS), contiene ácidos grasos saturados y monoinsaturados, así como también DHA. El perfil de seguridad de la BC-PS se determinó en varios estudios no clínicos. Estudios de seguridad por repetición de dosis en ratas y perros demuestran que la administración oral de la BS-PS en dosis de hasta 100 mg/kg/día durante 6 meses no mostró ningún efecto adverso significativo de interés toxicológico [12].
Los resultados de los estudios de teratogenicidad en ratas en dosis de hasta 200 mg/kg/día y en conejos en dosis de hasta 450 mg/kg/día mostraron que la administración oral de PS no afectó al desarrollo embrionario y fetal [12].
El potencial mutagénico de la BS-PS se ha investigado en varios tipos de células y no se revelaron hallazgos significativos. En un ensayo de micronúcleos, la BC-PS se administró a los ratones una dosis total de 30, 150 y 300 mg/kg en dos dosis iguales con 24 horas de separación. Los resultados del estudio no revelaron ninguna evidencia de potencial mutagénico o toxicidad en la médula ósea [12].
EXPERIENCIA CLÍNICA
Vayacog™ fue evaluado mediante un estudio aleatorio, de doble ciego, controlado con placebo, durante 15 semanas, seguido por una prórroga descubierta de 15 semanas más. En la fase de doble ciego, 157 hombres y mujeres (entre 50 y 90 años) con problemas de memoria fueron asignados aleatoriamente para recibir Vayacog™ o placebo (tres cápsulas al día) [13]. Durante la prórroga descubierta, todos los participantes tomaron una cápsula por día de Vayacog™.
El rendimiento cognitivo se evaluó mediante el Test de Aprendizaje Auditivo y Verbal de Rey (del inglés, AVLT de Rey) en el momento de inicio y al final (semana 15) de la fase de doble ciego y, utilizando una herramienta informática de evaluación cognitiva al inicio (semana 15) y al final (semana 30) de la fase de prórroga descubierta.
La valoración de la seguridad se evaluó a través del valoraciones por el laboratorio clínico, incluyendo parámetros bioquímicos y hematológicos en el inicio y el final (semana 15) de la fase de doble ciego y mediante el registro de los efectos adversos, la exploración física y medición de los signos vitales y del peso al inicio del estudio, en la semana 7, y al final (semana 15) de la fase de doble ciego y al final de la fase de prórroga descubierta (semana 30).

Methodology:
Study Design
This is a randomized, two-arm, double blind, multicenter two-stage adaptive Phase 3 study of paclitaxel and carboplatin with intravenous blinded placebo or intravenous blinded REOLYSIN® every 3 weeks (21 days is defined as a cycle). The study will be performed in the outpatient setting.
Population
Patients eligible for second line treatment of recurrent or metastatic squamous cell carcinoma of the head and neck (R/M SCCHN) who have progressed on or within 190 days after the last dose of a platinum based first line chemotherapy for R/M SCCHN, including an EGFR inhibitor such as cetuximab if approved and/or available for the patient. Eligible patients must not have been treated with a taxane for SCCHN and must have at least one lesion that is measurable by computed tomography (CT) or magnetic resonance imaging (MRI).
Stratification:
Patients enrolled will be stratified for: 1) ECOG performance status (0-1 and 2) and; 2) progression/relapse within 100 days vs. 101 to 190 days after prior platinum-based chemotherapy.
Number of Patients
It is anticipated that 80 evaluable patients will be treated in the first stage and between 100 and 400 in the second stage of the study. Enrollment into Stage 2 will continue at least until the assessment of Stage 1 is completed. The sample size in Stage 2 will be determined adaptively.
Dose Plan:
In Treatment Arm A:
On Day 1, paclitaxel - as a 3 hour intravenous infusion at a dose of 175 mg/m2.
Followed by:
Carboplatin - as a 30 minute intravenous infusion at a dose of AUC 5 mg/mL min calculated using Calvert’s formula (carboplatin dose (mg) = AUC x (calculated [by Cockcroft-Gault formula] CrCl (mL/min) 25)). Subsequently this calculation needs to be repeated only if serum creatinine increases by 10% over baseline. Note: Isotopic method to determine CrCl may be used if standard at a hospital (SEE IMPORTANT NOTE ABOVE REGARDING NEW DOSING GUIDELINES FOR CARBOPLATIN).
Followed by:
Blinded placebo (blinded saline from the pharmacy) over 1 hour.
On Days 2 through 5, ONLY blinded placebo will be administered.
In Treatment Arm B:
On Day 1, paclitaxel – as a 3 hour intravenous infusion at a dose of 175 mg/m2.
Followed by:
Carboplatin - as a 30 minute intravenous infusion at a dose of AUC 5 mg/mL min calculated using Calvert’s formula (carboplatin dose (mg) = AUC x (calculated [by Cockcroft-Gault formula] CrCl (mL/min) 25)). Subsequently this calculation needs to be repeated only if serum creatinine increases by 10% over baseline. Blinded REOLYSIN® administered as a 1 hour intravenous infusion at a dose of 3x1010 TCID50.
On Days 2 through 5, ONLY blinded REOLYSIN® will be administered, using the same dose and method as on Day 1.
The treatment cycle will be repeated every 21 days.
Tumor Evaluation
CT with contrast or MRI covering head and neck, lung and liver fields are required for all patients and will be collected for central review by an independent radiology review committee as further described in the study manual and a separate imaging charter. The tumors will be assessed following the revised RECIST Guidelines version 1.1 (Eisenhauer et al., 2009). Evaluation of tumor status will be conducted at baseline, at the end of week 6 on study and then every 6 weeks on and after study until disease progression, study termination, initiation of subsequent anticancer therapy, death, loss to follow-up or withdrawal of consent. Note, the scans should be performed on a calendar schedule (not to be affected by treatment delays or drug holidays).
Pharmacodynamic Endpoints
At some selected study sites, patients whose tumors are accessible to biopsy will be asked to undergo tumor biopsy (baseline and post therapy) to evaluate Ras pathway status, HPV status and to measure viral replication within the tumor. The timing of the second biopsy will vary, based on response, in order to optimise the data generated by this assessment.
Toxicity Requiring Dose Delay/Reduction
Dose delay/reduction is required if any one or more of the following events is determined to be possibly, probably or definitely related to paclitaxel and/or carboplatin and/or REOLYSIN® (as determined by the investigator). If treatment is resumed, the doses of therapy must be reduced even if the toxicity resolved (see Section 6.7 below).
• ANC < 0.5 x 109/L lasting for > 7 days,
• ANC < 0.1 x 109/L lasting for > 3 days, or
• ANC < 0.5 x 10 9/L with fever (>100.5º F or 38.1ºC).
• Platelet count < 25 x 109/L.
• Grade >2 cardiotoxicity, persistent Grade 2 neurotoxicity. Discontinue if > Grade 3 cardiotoxocity or neurotoxicity occurs.
• Any other drug-related non-hematological Grade 3/4 toxicity.
Management of Dose Reduction
Hematologic Toxicity
Patients experiencing any toxicity as described in Section 6.6 in any cycle will have their REOLYSIN® and/or paclitaxel and carboplatin treatments held until toxicity resolves to baseline or Grade 1. Upon resolution, REOLYSIN®, paclitaxel and carboplatin therapy may recommence at a lower dose level. Dose reduction of paclitaxel and/or carboplatin will be carried out according to the following guidelines:
• Reduction of carboplatin to AUC 4 and paclitaxel to 135 mg/m2 after:
ANC < 0.5 x 109/L lasting for > 7 days, or
ANC < 0.1 x 109/L lasting for > 3 days, or
ANC < 0.5 x 109/L with fever (>100.5º F or 38.1ºC).
• Reduction of carboplatin to AUC 4 mg/mL min after Grade 4 thrombocytopenia without neutropenia and paclitaxel to 135 mg/m2 after second occurrence of Grade 4 thrombocytopenia.
Cardiotoxicity or Neurotoxicity
• Reduction of paclitaxel to 135 mg/m2 for persistent Grade 2 peripheral neuropathy.
• The paclitaxel treatment will be discontinued if there is a Grade 3 or worse cardiotoxicity or peripheral neuropathy.
• Reduction of REOLYSIN® to 1x1010 TCID50 for Grade 2 cardiotoxicity and discontinuation of REOLYSIN® for Grade 3 or persistent Grade 2 cardiotoxicity.
Renal Impairment
If estimated GFR is below 30 mL/min, carboplatin is discontinued.

Metodología:
Diseño del estudio
Este es un estudio de paclitaxel y carboplatino en Fase 3, multicéntrico, de adaptación en dos etapas, aleatorio, de doble vertiente y con doble ciego, con placebo oculto intravenoso o REOLYSIN® oculto intravenoso cada 3 semanas (21 días se definen como un ciclo). El estudio se llevará a cabo en el ámbito ambulatorio.
Población
Pacientes elegibles para el tratamiento de segunda línea del carcinoma recurrente o metastático de células escamosas de cabeza y cuello (R/M CCECC) que ha progresado en o dentro de los 190 días después de la última dosis de quimioterapia para R/M CCECC, incluyendo un inhibidor de EGFR, tal como el cetuximab, si se aprueba/o está disponible para el paciente. Los pacientes elegibles no deben haber sido tratados con un taxano para CCECC y deben tener, al menos, una lesión medible mediante tomografía computerizada (TC) o resonancia magnética de imagen (RMI).
Estratificación:
Los pacientes inscritos serán estratificados según: 1) clasificación según el ECOG (0-1 y 2) y; 2) la progresión/recidiva dentro de los 100 días frente a 101 hasta 190 días después de la quimioterapia precedente basada en platino.
Número de Pacientes
Se prevé que 80 pacientes evaluables sean tratados en la primera etapa y entre 100 y 140 en la segunda etapa del estudio. La inscripción en la Etapa 2 continuará al menos hasta que la evaluación de la Etapa 1 se haya completado. El tamaño de la muestra en la Etapa 2 se determinará de forma adaptativa.

Seguido por:
Carboplatino – en forma de infusión intravenosa de 30 minutos a una dosis de ABC de 5 mg/mL•min calculada utilizando la fórmula de Calvert (dosis de carboplatino (mg) = ABC x (aclaramiento de creatinina calculado (en mL/min) 25) [por Cockcroft-Gault] ). Posteriormente este cálculo necesita ser repetido únicamente si la creatinina en suero se ve incrementada en un 10% respecto al valor basal. Nota: El método isotópico para determinar el aclaramiento de creatinina puede ser utilizado si está estandarizado en el hospital (VER NOTA IMPORTANTE CITADA ANTERIORMENTE SOBRE NUEVAS DIRECTRICES PARA LA DOSIS DE CARBOPLATINO).
Seguido por:
Placebo oculto (solución salina oculta de la farmacia) de encima de 1 hora hour.
Días 2 al 5, SOLAMENTE debe ser administrado el placebo oculto.

El ciclo del tratamiento se repetirá cada 21 días.
Evaluación del Tumor
TC con contraste o RMI cubriendo cabeza y cuello; los campos de pulmón e hígado son necesarios para todos los pacientes y se recogerán para su revisión central por un comité independiente de revisión radiológica, como se describe en el manual de estudio y en la documento de imágenes separado. Los tumores serán evaluados según las Directrices RECIST revisadas con versión 1.1 (Einsenhauer et al., 2009). La evaluación del estado del tumor se llevará a cabo, como referencia, al final de la semana 6 de estudio y luego, cada 6 semanas a partir del estudio hasta la progresión de la enfermedad, finalización del estudio, inicio de posteriores terapias antitumorales, fallecimiento, la pérdida de seguimiento o retirada de consentimiento. Debe tenerse en cuenta que las exploraciones deben realizarse en un calendario programado (no debe estar afectado por retrasos en el tratamiento o pausas en la administración de los fármacos).
Aspectos Farmacodinámicos
En algunos sitios de estudio seleccionados, los pacientes cuyos tumores son accesibles a la biopsia, se les pedirá someterse a una biopsia del tumor (línea de base y tratamiento posterior) para evaluar el estado de la vía de Ras, el estado de VPH y para determinar la replicación viral en el interior del tumor. El momento de la segunda biopsia puede variar, basado en la respuesta, a fin de optimizar los datos generados por esta evaluación.
Toxicidad que Requiera Retardo/Reducción de Dosis
El retraso/reducción de dosis es necesaria si uno o más de los siguientes eventos se determina como posible, probable o definitivamente relacionado con el paclitaxel y/o con el carboplatino y/o el REOLYSIN® (según lo determinado por el Investigador). Si el tratamiento se reanuda, la dosis de la terapia debe ser reducida, aunque la toxicidad se haya resuelto (véase Sección 6.7 más adelante).
• RAN < 0.5 x 109/L durante > 7 días,
• RAN < 0.1 x 109/L durante > 3 días, o
• RAN < 0.5 x 10 9/L con fiebre (>100.5º F o 38.1ºC).
• Recuento de plaquetas < 25 x 109/L.
• Cardiotoxicidad de Grado ≥ 2, neurotoxicidad persistente de Grado 2. Suspender si tiene lugar una cardiotoxicidad o neurotoxicidad de Grado ≥ 3.
• Cualquier otra toxicidad no hematológica de Grado 3/4 relacionada con los fármacos.
Gestión de la Reducción de la Dosis
Toxicidad Hematológica
Los pacientes que experimenten una toxicidad en cualquier ciclo tal, y como se describe en la Sección 6.6, tendrán suspendido su tratamiento con REOLYSIN® y/o paclitaxel y carboplatino hasta que la toxicidad vuelva a la línea de base o sea de Grado 1. Tras la resolución, la terapia con REOLYSIN®, paclitaxel y carboplatino puede reanudarse con una dosis más baja. La reducción de la dosis de paclitaxel y/o carboplatino se llevará a cabo de acuerdo con las siguientes pautas:
• Reducción de carboplatino a un ABC de 4 y de paclitaxel a 135 mg/m2 una vez que:
RAN < 0.5 x 109/L durante > 7 días, o
RAN < 0.1 x 109/L durante > 3 días, o
RAN < 0.5 x 109/L con fiebre (>100.5º F or 38.1ºC).
• Reducción de carboplatino a un ABC de 4 mg/mL min después de una trombocitopenia sin neutropenia de Grado 4 y de paclitaxel a 135 mg/m2 después de la segunda aparición de una trombocitopenia de Grado 4.
Cardiotoxicidad o Neurotoxicidad
• Reducción de paclitaxel a 135 mg/m2 para la neuropatía periférica persistente de Grado 2.
• El tratamiento con paclitaxel se suspenderá si hay una cardiotoxicidad o neuropatía periférica de Grado 3 o más grave.
• Reducción de REOLYSIN® a 1x1010 DICT50 para la cardiotoxicidad de Grado 2 e interrupción de REOLYSIN® para una cardiotoxicidad de Grado 3 o de Grado 2 persistente.
Insuficiencia Renal
Si el FG estimado es inferior a 30 ml/min, el carboplatino debe interrumpirse.

Gestión de la Reducción de la Dosis
Toxicidad Hematológica
Los pacientes que experimenten una toxicidad en cualquier ciclo tal, y como se describe en la Sección 6.6, tendrán suspendido su tratamiento con REOLYSIN® y/o paclitaxel y carboplatino hasta que la toxicidad vuelva a la línea de base o sea de Grado 1. Tras la resolución, la terapia con REOLYSIN®, paclitaxel y carboplatino puede reanudarse con una dosis más baja. La reducción de la dosis de paclitaxel y/o carboplatino se llevará a cabo de acuerdo con las siguientes pautas:
• Reducción de carboplatino a un ABC de 4 y de paclitaxel a 135 mg/m2 una vez que:

Doses of 1 g or 2 g are utilised in the vast majority of studies in all indications. Dosing in earlier studies was often eight hourly but more recent studies have demonstrated the adequacy of twelve hourly dosing. In very severe, intractable infections such as meningitis higher and more frequent dosing is more usually implemented. This range of dosing is fully addressed in the SPC for Cefotaxime BIOCHEMIE.
Similarly, the efficacy of cefotaxime as a single 1 g dose in the treatment of gonorrhoea is confirmed by the clinical studies. Also the use of cefotaxime as prophylaxis of peri- or post-operative infections, has been extensively researched. In this indication, many studies have utilised cefotaxime pre-, peri-, and post-operatively, but more recent studies support the use of a single 1 g or 2 g pre-operative dose. Thus these indications are justifiably included in the SPC for Cefotaxime BIOCHEMIE, and the specific dosing regimens specified in the SPC are appropriate.
The treatment of abdominal infections with cefotaxime 1 g every 12 hours when used in combination with another antibiotic such as metronidazole has been shown to be extremely effective, and thus inclusion of this indication in the SPC for Cefotaxime BIOCHEMIE is warranted. The amount of data on bone and joint infections is limited, however, there is evidence of efficacy in this indication and given the relatively good tissue penetration of cefotaxime into bone, its inclusion in the SPC is justified.
The optimal treatment for Lyme’s borreliosis has not yet been identified, however the few clinical trial available indicate that cefotaxime has a role in this indication, particularly when it is penicillin resistant. The dosage utilised has been 2 g three times daily and thus, this Cefotaxime is one of the recommended antibiotics for the treatment of endocarditis caused by the HACEK range of micro-organisms. The dosage recommended corresponds to cefotaxime 1 g twice daily, the inclusion in the SPC for Cefotaxime BIOCHEMIE, therefore, appropriate.indication can legitimately be included in the SPC for Cefotaxime BIOCHEMIE.

Background
•Due to internal organ displacement caused by peristalsis, for example, certain target volumes such as tumors of the prostate or liver may be difficult to localize using an external reference system.
•For this reason, implanted fiducial markers may be used to facilitate appropriate patient positioning in relation to the linac isocenter when treating mobile target volumes.
•Various marker types are available. ExacTrac supports both long and short markers (see “Handling Implanted Markers” on page 140).
•Even if the position of the target volume in relation to fixed anatomical structures or body markers changes, the target volume can still be accurately tracked if implanted markers are used.
Recommendations
Implanted markers should be positioned inside, or as close as possible to, the target volume:
•If only one marker is used, rotations cannot be determined and marker migration may go unnoticed.
•Three implanted markers should therefore be used to allow the system to calculate rotations and detect any changes in the spatial relations between individual markers.
•Using more than three markers increases the risk of one marker obscuring the other in the acquired images.
•Implanted markers must be sufficiently visible in the acquired images. To verify visibility, first acquire test x-ray images of a phantom with the relevant markers attached.
Risks
To prevent migration of implanted markers reducing treatment accuracy, marker implantation should be performed at least one week prior to the acquisition of the CT dataset used for treatment planning.
Depending on the region where markers are to be implanted, non-migrating markers should be used. The hospital or clinic is responsible for choosing a suitable marker pattern.
Implanting fiducial markers is a complicated and potentially hazardous procedure that must be performed by specialized personnel with appropriate training. The choice of surgical procedure and implanted marker is the responsibility of the hospital or clinic. BrainLAB does not offer guidance on specialized surgical procedures of this kind.
Regulatory Considerations
Local regulations (for example FDA approval or specific regulatory guidelines) must be observed when using implanted fiducial markers.
Patient Positioning on the Couch
Before You Begin
To prevent injury to patients wearing pacemakers, contact the pacemaker manufacturer regarding possible hazards resulting from radiation exposure before commencing patient treatment.
General Positioning Recommendations
• Ensure that the patient is not positioned near any metal components in the couch top. Otherwise, these components will be included as artifacts in subsequent x-ray correction and verification images.
• To enable x-ray image acquisition, also ensure that the field of view between the x-ray tubes and the flat panels is not obstructed.
Recommendations for Gated Treatments
Gating refers to the technique of synchronizing the linac beam with the breathing cycle of the patient. The goal is to irradiate the patient only when the tumor intersects with the linac beam at isocenter. For this reason, gated treatments require the full cooperation of the patient. It is absolutely critical that the patient does not move during the procedure. The arms especially should be kept absolutely still. Use of a patient fixation system such as the BodyFIX vacuum positioning system may be helpful in achieving optimal precision.

The application of computer models for clinical and engineering problems in the fields of orthopedics and trauma surgery is of increasing interest. The so-called Finite Element Method (FEM) provides the possibility to capture the whole mechanical situation of a structure. In contrast, data in real experiments is limited and the quantity and particularly the location of the parameters of interest have to be identified beforehand. Furthermore, virtual experiments can be repeated arbitrarily. For example, the effect of model parameters (e.g. implant geometry) on dependent values (e.g. stiffness) can be examined using this approach. Simulation models in this context can be prepared from computed tomography (CT) imaging data with manageable effort. Hence, numerical simulation can be utilized in the future to assist presurgical planning by checking mechanical properties of different treatment options.
Similar to procedures in real experiments, it is necessary to make certain assump-tions because the complexity of real structures can be neither entirely described nor modeled in full detail. In the scope of the musculoskeletal system, for example, only a simplified configuration of acting muscle forces is usually considered, both for real and virtual experiments. This step is commonly denoted as „physical modeling“ and often represents the most challenging part during the design phase of experiments. In this process it is imperative that the information of the target parameter is not inva-lidated through the model assumptions. In addition to the idealization of the testing environment it is necessary in virtual modeling to represent the morphology and ma-terial behavior of bone and implant as accurate as possible.
The following tutorials present calculation results of different sample models. Various influences on the mechanical stability of special osteosynthesis treatments are studied on the basis of fractured long bones with the help of numerical simulation. In particular, mechanical characteristics of fixed fractures, implant-specific properties as well as influences due to modifications in implant application are discussed.
To allow users from disciplines besides engineering sciences to follow the analysis of the examples subsequently, the module starts with a short introduction to common mechanical measures.

Además de la idealización del entorno del ensayo, en el modelado virtual, es necesario representar la morfología y el comportamiento material del hueso y del implante de la forma más precisa posible.
Los tutoriales siguientes muestran los resultados del cálculo de diferentes modelos de muestra. Se estudian, con la ayuda de una simulación numérica, diversas in-fluencias en la estabilidad mecánica de los tratamientos especiales de osteogénesis en base a huesos largos fracturados. En particular, se discuten las características mecánicas de las estructuras fijadas, las propiedades específicas de los implantes, así como también las influencias debidas a modificaciones en la aplicación del im-plante.
